Additional Eligibility Requirements. An institution may impose additional requirements for enrollment in courses for dual credit that do not conflict with 19 Administrative Code 4.85. 19 TAC 4.85(b)
Additional Eligibility Requirements. 1. To be eligible for the increase set forth in 23.1A(1), (2) and (3), employees must have been employed on or before August 7, 2016, August 7, 2017 and August 7, 2018, respectively.
2. Employees who are on visiting appointments, temporary appointments or who have received notice of non-reappointment or dismissal or who have resigned with an effective date occurring prior to the effective date of the salary increase described herein are not eligible to receive the increase described in Article 23.A1.

You must comply with the following requirements, or your application will be screened out and will not be reviewed: use of the PHS 5161-1 application form; application submission requirements in Section IV-3 of this document; and formatting requirements provided in Appendix A of this document.

If there is no proof indicating the purchase date, the manufacture date shall be considered as the beginning of the Guarantee Term or Warranty Term.

To be eligible for the merit base increases set forth in Sections 23.1A1, 23.1B1, and 23.1C1(1), (2), and (3), employees must have been employed on or before August 7, 2021, August 7, 2022, and August 7, 2023, respectively, and must be employed on the date the increase is paid. 2016, August 7, 2017 and August 7, 2018 respectively.

Employees who are on visiting appointments, temporary appointments or who have received notice of non-reappointment or dismissal or who have resigned with an effective date occurring prior to the effective date of the merit base salary increases described herein in this Article are not eligible to receive the increases described in Article 23.
Additional Eligibility Requirements. 1. To be eligible for the merit base increases set forth in Sections 23.1B1, and 23.1C1employees must also have been continuously employed from on or before, August 7, 2022, and August 7, 2023, respectively, and must be employed on the date the increase is paid.
2. Employees who are on visiting appointments, temporary appointments or who have received notice of non-reappointment or dismissal or who have resigned with an effective date occurring prior to the effective date of the merit one-time lump sum bonus and merit base salary increases described in this Article are not eligible to receive the bonus and increases described in this Article.
Additional Eligibility Requirements. In addition to the basic eligibility requirements in Section 2.2.A., above, a Member must meet the additional eligibility requirements that are listed in the Group Contract Application. The Group is required to administer these requirements in strict accordance with the terms that have been agreed to and cannot change the requirements or make an exception unless CareFirst approves them in advance, in writing.
Additional Eligibility Requirements. In addition to any other eligibility criteria provided for in the Plan, the Company may require that the Team Member execute a separate document agreeing to the terms of a current arbitration agreement and/or a current ARCIPD, each in a form acceptable to the Company and/or that the Team Member be in compliance with the ARCIPD throughout the entire Restriction Period and through the date the XXXXX is to be granted or settled. If such separate documents are required by the Company and the Team Member does not accept them within 75 days of the Grant Date or such other date as of which the Company shall require in its discretion, this XXXXX shall be canceled and the Team Member shall have no further rights under this Grant Agreement.
