ADDRESS LIST. CC.01 The Employer will provide the Union with a list of phone numbers and addresses of all employees in the bargaining unit April 1st and October 1st of each year.
ADDRESS LIST. The Hospital will provide the Union every April upon written request, with a list of addresses of all employees in the bargaining unit. Employees not wishing to have this released to the Union, must inform the Hospital in writing.

ADDRESS LIST. A current address list of all employees will be provided to the Union every six (6) months. The list will be provided in hard copy or on a diskette, whichever is requested.
ADDRESS LIST. Upon ratification of this Agreement and every twelve months thereafter, or as otherwise agreed, the Company shall provide the Chief Xxxxxxx with a list of all employees covered by this Agreement, showing names, addresses and postal codes, as currently indicated in the Company records.
ADDRESS LIST. The Employer shall provide the Union with a quarterly list of employee addresses and phone numbers. COPY OF AGREEMENTS The Employer shall provide all employees with a copy of the collective agreement, and updated insurance booklet. Copies of the current company policies shall be provided when available. CLASSIFICATION RATES OF PAY If an employee bids to a classification with a higher rate of pay he/she shall receive an increase equivalent to the difference in base rates between his/her existing position and the new position. If an employee bids to a classification with a lower rate of pay his/her wage rate shall be reduced by the difference between the base rates of his/her existing position and the new position.
ADDRESS LIST. The Subdivider shall submit a printed and electronic list, showing in numerical order by Assessor’s Parcel Number, the names and mailing addresses of the last known owners of all properties touching or within the 300-foot radius of the subdivision shown on the map. These names and addresses are available at the Tax Collector’s Office and are shown on the latest city-wide assessment roll. The list also shall include the names of the residents within the subdivision itself. In addition, the Subdivider shall include on the list names and addresses of the persons, organizations, or any other agencies that have made a formal request to the Department to receive notification concerning the subdivision. The electronic copy of the list shall be in *.xls or *.txt format or other format acceptable to the Director.
ADDRESS LIST. 43 Employees on layoff status shall file their addresses in writing with the Personnel Office of the 44 District and shall thereafter promptly advise the District in writing of any change of address.
