Whilst the separate elements of the service are contracted for separately, it is a term of the providerscontracts that they collaborate with each other via a Provider Management Board. Whilst the providers are responsible for delivery distinct elements of each service (and these are set out in the specifications and PMB agreements) they are also collectively responsible for delivering a seamless and integrated service to patients. The members of the Provider Management Board will be held to account in terms of service delivery by a quarterly Performance and Quality Board chaired by joint commissioners from CCG and BHCC. The effectiveness of the Provider Management Board as a vehicle for delivering an integrated service according to the service specifications will be evaluated on an annual basis. SCHEDULE 6

STS includes the following elements: • Bed based and community STS (formerly intermediate care) • Integrated rapid response service (including Community Rapid Response, Roving GP, OOHs District Nursing and Age UK Crisis)

A specific work programme has been undertaken to create a responsive and flexible business model with significantly greater GP involvement and leadership. The revised ACU Management Committee led by pathfinders meets on a monthly basis. Cluster wide functions The Cluster has established a number of functions that provide support across the borough teams as well as to the corporate arrangements. These include finance, performance, informatics, strategy and HR/OD as well as co-ordination of the transition programme Commissioning support development SW London is working with its pathfinders to develop sustainable commissioning support arrangements. This work is being undertaken within the NHSL commissioning support programme. OD/HR The Borough MDs are also responsible for supporting the development of pathfinders locally. Specialist OD and HR support and expertise is available from the HR/OD and Workforce directorate. This will primarily focus on supporting the commissioning, procurement and contract management of the MPET funded support with much of the OD and leadership support being provided by independent organisations. We have also been providing advice on JDs, remuneration etc. as requested.

  • Governance (a) The HSP represents, warrants and covenants that it has established, and will maintain for the period during which this Agreement is in effect, policies and procedures:
