Credit Monitoring and Insurance Services. If you wish to receive Credit Monitoring and Insurance Services, you must check off the box for this section, provide your email address in the space provided in Section II, above, and return this Claim Form. Submitting this Claim Form will not automatically enroll you into Credit Monitoring and Insurance Services. To enroll, you must follow the instructions sent to your email address after the Settlement is approved and becomes final (the “Effective Date”).
Credit Monitoring and Insurance Services. Two years of Credit Monitoring and Insurance Services (“CMIS”). CMIS will include credit monitoring, fraud consultation, and identity theft restoration services. Class Members who already maintain a credit monitoring service may elect to defer their enrollment in the CMIS for a period of 12 months for no additional charge. The CMIS will include the following services, among others, to be provided to each Class Member who submits a valid claim for CMIS: (i) up to $1 million dollars of identity theft insurance coverage; and (ii) three-bureau credit monitoring providing notice of changes to the Class Member’s credit profile.
Credit Monitoring and Insurance Services. “CMIS”). In addition to the Out-of-Pocket Losses Payment (or in alternative to the Cash Fund Payment), Class Members may elect to claim three years of CMIS to be provided by a vendor agreed upon by the parties and paid for out of the Settlement Fund. The CMIS benefit will provide, at a minimum, one bureau credit monitoring services; dark web scanning with immediate user notification if potentially unauthorized use of a class member’s personal information is detected; one million dollars in identity theft insurance; real-time credit monitoring; and access to fraud resolution agents. Said CMIS benefits will be available to class members irrespective of whether they took advantage of any previous offering of credit monitoring from ReproSource. Individuals who elected to utilize a previous offering of CMIS from ReproSource, or who obtained CMIS services from another provider as a result of the Security Incident, will be permitted to postpone activation of their CMIS settlement benefit for up to 12 months. CMIS benefits are available to Class Members who submit a claim for Out-of-Pocket Losses (including Lost Time) under the Settlement; however, Class Members may only elect to receive either CMIS benefits or a Cash Fund Payment.
Credit Monitoring and Insurance Services. Each Settlement Class Member who submits a valid and timely Claim Form may elect to receive three (3) years of Credit Monitoring and Insurance Services (“CMIS”) regardless of whether they also make a claim for a Settlement Payment pursuant to Paragraph 28 above. The CMIS will include up to $1 million of identity theft insurance coverage and three bureau credit monitoring.

In the alternative to the Documented Loss Payment or the Cash Fund Payment, class members may elect to claim 12 months of CMIS to be provided by a vendor agreed upon by the parties. Said CMIS benefits will be available to class members irrespective of whether they took advantage of any previous offering of credit monitoring from Hope College. Individuals who elected to utilize a previous offering of CMIS from Hope College, or who obtained CMIS services from another provider as a result of the Data Breach, will be permitted to postpone activation of their CMIS settlement benefit for up to 12 months.
Credit Monitoring and Insurance Services. In addition to or instead of selecting one of the above cash Settlement Payment options, you also may submit a Claim Form for 12 months of free Credit Monitoring and Insurance Services provided by TransUnion, which includes three credit bureau monitoring services and $1 million in identity theft insurance.
Credit Monitoring and Insurance Services. 4.6.1 In addition to the benefits and distributions from the Net Qualified Settlement Fund as described above in ¶¶ 4.3–4.5, all Settlement Class Members will also be eligible to enroll in two years of Credit Monitoring and Insurance Services. Because some Settlement Class Members may have existing credit monitoring services, a Settlement Class Member may elect to delay the start of the Credit Monitoring and Insurance Services received under this section for up to two years after the Effective Date. The Credit Monitoring and Insurance Services will provide certain services to each Participating Settlement Class Member, including:
(i) Up to One Million Dollars and No Cents ($1,000,000.00) in reimbursement insurance covering future losses due to identity theft, stolen funds, etc.; (ii) three bureau credit monitoring providing notice of changes to the Settlement Class Member’s credit profile; (iii) real time instant authentication alerts when someone attempts to make a change to the Settlement Class Member’s personal account information; (iv) Dark Web Monitoring providing notification if the Settlement Class Member’s information such as Social Security number, credit card numbers, financial account numbers, and health insurance number are found on the Dark Web; (v) threat alerts about potential threats relevant to the Settlement Class Member (such as breaches, phishing scams, and malware vulnerabilities); and (vi) customer support and victim assistance, among other features.

Each Settlement Class Member is eligible to submit a claim for two (2) years of three-bureau credit monitoring and up to $1,000,000 of insurance coverage for identity theft incidents. The Credit Monitoring and Insurance Services (“CMIS”) option will include credit monitoring, fraud consultation, and identity theft restoration services.

The CMIS will include the following services to be provided to each Settlement Class Member who submits a valid and timely Claim Form and elects the CMIS: (i) up to $1 million dollars of identity theft insurance coverage; (ii) three bureau credit monitoring providing notice of changes to the Settlement Class Members’ credit profile;

Each Settlement Class Member who submits a valid and timely Claim Form may elect to receive three (3) years of three-bureau CMIS regardless of whether they also make a claim for a Settlement Payment. • Included in this settlement as Settlement Class Members are: o All residents of the United States who were sent notice that their personal information may have been accessed, stolen, or compromised as a result of the Data Incident. o Excluded from the Settlement Class are: (i) Defendant and its respective officers and directors; (ii) any Person who would otherwise be a member of the Settlement Class but who timely and validly requests exclusion from the Settlement Class; (iii) the Judge and Magistrate Judge assigned to evaluate the fairness of this settlement, the Court’s staff, and the Court’s immediate family members; and (iv) any other Person found by a court of competent jurisdiction to be guilty under criminal law of initiating, causing, aiding, or abetting the Data Incident or who pleads nolo contendere to any such charge.
