Current Information System. The current health information system (HIS) includes medical assistants (Feldshars) working in FAPs, and nurses and midwives working at Family Group Practitioner Centers. These personnel are responsible for submitting a detailed monthly report on their specific maternal and child health activities and cases. For maternal health, the data collected includes the name of each woman who delivered a baby in the catchment area, the age of the woman, date of childbirth, location of childbirth, gender of child, birth registration number from the local administration office, the date of the registration, the date of previous delivery, the name of the child and its birth weight. This information is compiled by the Chief family practitioners at the end of the month, and brought into the Health Information Center located at the Rayon’s Family Medicine Center. The Information Center’s HIS team (3 in the Aksy Rayon and 2 in Bazarkorgan), sits down with each family practitioner and goes over all the data and each report very carefully. Sometimes the Feldshar comes in with the family practitioner to discuss some of the information being submitted. For child health, immunization data on each child is kept in a family notebook, compiled and submitted directly by the nurse or midwife provider, and not by the supervising family practitioner – although the FGP does check this information before it is submitted. This information is submitted to the Rayon Sanitation and Epidemiology Station, not to the Information Center. For the outpatient treatment of the ill child or adult, the family practitioners complete a report on client visit and treatment provided. These particular reports are also sent on to the Information Center at the end of each month for health insurance coverage purposes. The FGP does not retain a copy at the center. This information is retained within a visit notebook at the center where service was rendered. The next level of reporting is done by the Rayon Health Information Center and Rayon Sanitation and Epidemiology Station. The prior produces a Quarterly Report compiling the 3-month activity per Chief FGP. This information includes the total number of births, number of deaths, number of deaths under age 14, number of deaths under 1 year, number of deaths under 2 years, mortality ratio per 1000 live births, early neonatal deaths (within the first 24 hours), and the number of deaths in the home. Current Information System. There is a health information system in the target area, though it does not sufficiently generate quality data. At the community level, there are community own resource persons (CORPS) trained by BASICS II and PDCs trained by UNICEF. On a quarterly basis, they collect multi- sectoral data that include latrine coverage, water and sanitation, housing status, school children enrollment, immunization coverage, agriculture and household income. Over the last one year, the UNICEF country program introduced an additional tool to capture data on deaths/births for under-fives. This information is compiled on a monthly basis. The data collected are used to inform the district on key household practices, mortality and morbidity trends including community critical needs to use for planning. During phase I, in partnership with DISH II Project, the project focused on training health workers in data collection, interpretation/ analysis and use but there was no refresher training and follow up and support supervision. This was supposed to be done by DISH II, which phased out before end of the CIMCI project thus leaving some gaps in the HIS which CIMCI Plus will address. At the health facility level, the Ntungamo Health Services uses the national MOH recommended HMIS forms. The forms capture data on care seeking of major diseases including those that relate to childhood illnesses (malaria, diarrhea, malnutrition, ARI and HIV-AIDS), health services utilization and human resources. Health workers collect data and send it to the health sub-district level. From the sub-district, it is sent to the district for basic analysis by the HMIS Officer and then to the MOH for detailed analysis and aggregation. On some occasions, health workers are provided with some skills to be able to use the data in decision-making. However, the health workers’ skills are not adequate enough and yet information use is also affected by delayed feedback at the national level. In addition to the HMIS data, the district carries out weekly diseases surveillance on notifiable and epidemic potential diseases to avoid possible widespread of epidemics. There is also on going health facility assessment by the MOH with support from WHO on an annual basis.   • Current Information (a) During the period from the date of this Agreement to the Closing, each Party hereto shall promptly notify each other Party of any (i) significant change in its ordinary course of business, (ii) proceeding (or communications indicating that the same may be contemplated), or the institution or threat or settlement of proceedings, in each case involving the Parties the outcome of which, if adversely determined, could reasonably be expected to have a material adverse effect on the Party, taken as a whole or (iii) event which such Party reasonably believes could be expected to have a material adverse effect on the ability of any party hereto to consummate the Share Exchange. (b) During the period from the date of this Agreement to the Closing, NAS shall promptly notify OTM of any correspondence received from the SEC and FINRA and shall deliver a copy of such correspondence to OTM within one (1) business day of receipt.

  • Access to Information Systems Access, if any, to DXC’s Information Systems is granted solely to perform the Services under this Order, and is limited to those specific DXC Information Systems, time periods and personnel as are separately agreed to by DXC and Supplier from time to time. DXC may require Supplier’s employees, subcontractors or agents to sign individual agreements prior to access to DXC’s Information Systems. Use of DXC Information Systems during other time periods or by individuals not authorized by DXC is expressly prohibited. Access is subject to DXC business control and information protection policies, standards and guidelines as may be modified from time to time. Use of any other DXC Information Systems is expressly prohibited. This prohibition applies even when an DXC Information System that Supplier is authorized to access, serves as a gateway to other Information Systems outside Supplier’s scope of authorization. Supplier agrees to access Information Systems only from specific locations approved for access by DXC. For access outside of DXC premises, DXC will designate the specific network connections to be used to access Information Systems.

  • Information Systems The Customer is aware that vehicles manufactured, supplied or marketed by a company within the Volvo Group are equipped with one or more systems which may gather and store information about the vehicle (the “Information Systems”), including but not limited to information relating to vehicle condition and performance and information relating to the operation of the vehicle (together, the “Vehicle Data”).   • Student Information In the course of providing services during the term of the contract, certain personnel of Consultant may have access to student education records that are subject to the Family Educational Rights and Privacy Act (FERPA), 20 U.S.C. 1232g, et seq. and the regulations promulgated there under. Such information confidential and is therefore protected. To the extent that Consultant’s personnel require access to “education records” to perform Services pursuant to this Agreement, such personnel are deemed a “school official,” as each of these terms are defined under FERPA. Consultant agrees that it shall not use education records for any purpose other than in the performance of this contract. Except as required by law, Consultant shall not disclose or share education records with any third party unless permitted by the terms of the contract or to subcontractors who have agreed to maintain the confidentiality of the education records to the same extent required of Consultant under this contract. For the avoidance of doubt, District will be responsible for obtaining any necessary consents from students or parents pursuant to FERPA to provide the information to Consultant. In the event any person(s) seek to access protected education records, whether in accordance with FERPA or other Federal or relevant State law or regulations, the Consultant will immediately inform the District of such request in writing if allowed by law or judicial and/or administrative order. Consultant shall not provide direct access to such data or information or respond to individual requests. Consultant shall only retrieve such data or information upon receipt of, and in accordance with, written directions by the District and shall only provide such data and information to the District. It shall be District’s sole responsibility to respond to requests for data or information received by Vendor regarding District data or information. Should Consultant receive a court order or lawfully issued subpoena seeking the release of such data or information, Consultant shall provide immediate notification to the District of its receipt of such court order or lawfully issued subpoena and shall immediately provide the District with a copy of such court order or lawfully issued subpoena prior to releasing the requested data or information, if allowed by law or judicial and/or administrative order. If Consultant experiences a security breach concerning any education record covered by this contract, then Consultant will immediately notify the District and take immediate steps to limit and mitigate such security breach to the extent possible. The parties agree that any breach of the confidentiality obligation set forth in the contract may, at District’s discretion, result in cancellation of further consideration for contract award and the eligibility for Consultant to receive any information from District for a period of not less than five (5) years. In addition, Consultant agrees to indemnify and hold the District harmless for any loss, cost, damage or expense suffered by the District, including but not limited to the cost of notification of affected persons, as a direct result of the unauthorized disclosure of education records. Upon termination of Agreement, Consultant shall return and/or destroy all data or information received from the District upon, and in accordance with, direction from the District. Consultant shall not retain copies of any data or information received from the District once the District has directed Consultant as to how such information shall be returned to the District and/or destroyed. Furthermore, Consultant shall ensure that they dispose of any and all data or information received from the District in a District-approved manner that maintains the confidentiality of the contents of such records (e.g. shredding paper records, erasing and reformatting hard drives, erasing and/or physically destroying any portable electronic devices).

  • Patient Information Each Party agrees to abide by all laws, rules, regulations, and orders of all applicable supranational, national, federal, state, provincial, and local governmental entities concerning the confidentiality or protection of patient identifiable information and/or patients’ protected health information, as defined by any other applicable legislation in the course of their performance under this Agreement.
