Dental Insurance – Active Employees. The District will provide Delta Dental Insurance. The District will pay sixty (60%) percent of the cost of the premiums and the employees will pay forty (40%) percent. Additional riders for dental insurance may be selected by teachers, provided that the entire cost of any such rider must be paid for by the teacher.

Dental Insurance – Active Employees. The City shall continue to pay the full premium costs of Delta Dental Plan C coverage for each employee and his/her family. Plan C provides fifty percent (50%) of treatment costs of Class I and Class II benefits with an eight hundred dollar ($800) maximum per person per contract year. Effective July 1, 1988, orthodontic coverage will be provided, covering fifty percent (50%) of costs up to a lifetime maximum of $1,000. Effective July 1, 2001, the lifetime maximum for orthodontic coverage will be increased to $2,500. Coverage under this plan is afforded to each employee who is a member of this bargaining unit and his/her dependents. When an employee and spouse are both employed by the City and eligible for coverage, dental benefits shall be coordinated in accordance with the policy of the insurance carrier.
Dental Insurance – Active Employees. The Employer shall pay the full premium costs of Delta Dental Plan C coverage for each employee. Plan C provides 50% of treatment costs on Class I and Class II Benefits, with an $800.00 maximum per person per insurance contract year. Effective July 1, 2003, the yearly maximum for Class I and Class II Benefits will increase to twelve hundred dollars ($1,200) per year for active employees. Coverage under the plan is afforded to the subscriber (employee) and the subscriber's dependents. Class I Benefits include: Basic Services, Preventive Services, Restorative Services, Oral Surgery Services, Endodontic Services, and Periodontic Services. Class II Benefits include: Prosthodontics Services. Effective July 1, 1988, active employees and their covered dependents will also be provided orthodontic coverage for 50% of treatment costs, with a $1,000.00 lifetime maximum per person. Effective July 1, 2003, the lifetime maximum for orthodontic coverage shall increase to three thousand dollars ($3,000) per person. Alternative dental coverage through Midwestern Dental will be offered to employees in the first regular open enrollment following ratification of the 2001 agreement.

  • ’ Compensation/Employer’s Liability Insurance If Contractor has employees, it shall maintain workers’ compensation insurance as required by law. Employer’s liability limits shall be not less than $1,000,000 for each accident, $1,000,000 as the aggregate disease policy limit, and $1,000,000 as the disease limit for each employee. If Contractor does not have employees, it shall provide a letter, on company letterhead, to the Judicial Council certifying, under penalty of perjury, that it does not have employees. Upon the Judicial Council’s receipt of the letter, Contractor shall not be required to maintain workers’ compensation insurance.

  • Retiree Health Insurance Retired members of the Department receiving, or to receive City of Lincoln monthly pension checks, may participate in the group comprehensive health care plan for active City employees, provided that each retiree so desiring will execute the required forms in a timely fashion, and further provided that each retiree will be required to pay the full monthly cost at the current rates subject to any rate increases which may occur from time to time. Such payment will be made by payroll deduction from pension checks, or by direct payment in the case of an early retiree.

  • Dental Insurance The State agrees to pay one hundred percent (100%) of the employee premium of a dental insurance program for full-time employees. The benefit levels of this program shall provide one hundred percent (100%) coverage for preventive care and eighty percent (80%) coverage for general service care. The State agrees to provide payroll deduction for dental insurance, provided such arrangements are agreed to by the insurance carrier. Dependent coverage will be available provided there is sufficient employee participation in the dental insurance program. Dependent coverage will be at the employees' expense.
