DESCRIPTION OF PROGRAM SERVICES. A. SRPD will provide the services described herein (“Services”):
1. Provide staff to operate the program as described in the ELOP guidelines attached hereto and hereby incorporated by reference as Exhibit “A” ELOP Guidelines.
2. Provide supervision of the personnel employed under this Agreement. All terms of employment of said persons, including working conditions, discipline, hiring and discharging, or other terms of employment and/or requirements of law, shall be made by the SRPD and DISTRICT shall have no right or authority over such persons or the terms of such employment; however, DISTRICT will have input on the selection and placement of the staff hired for this program.
3. Delivery of Services will occur during before school and after school program hours and times vary from 7:00 a.m. to 6:00 p.m. on designated days of program operation. Before School: 7:00 a.m. – 9:25 a.m. (M – F) After School: 12:50 p.m. – 6:00 p.m. (M – F) After School: 2:40 p.m. – 6:00 p.m. (M) After School: 3:40 p.m. – 6:00 p.m. (T – F)
4. Be responsible for maintaining evaluation data, attendance forms, and other relevant program documentation as needed by program.
5. Provide appropriate recreational, educational and custodian supplies for services that will be billed to DISTRICT under terms of reimbursable payment.
B. DISTRICT agrees to the following:
1. Provide appropriate space and general operational supplies for Services. This space will include access to needed school facilities, office equipment, and supplies. Overall Facility Maintenance will be handled by the district who owns the facility.
2. Develop common confidentiality guidelines to share information between DISTRICT and SRPD to the extent permitted by the California Education Code and the Welfare and Institutions Code governing client confidentiality.
3. Work collaboratively with SRPD to identify and meet the needs of students served in the program.
4. Set meeting and training schedules.
5. Maintain coordination with other agencies and services providers with SRPD.
6. Provide the needed audit documentation to the state as required by the ELOP guidelines.
DESCRIPTION OF PROGRAM SERVICES. All services required to be performed by a participating servicer as set forth in the Program Documentation for the Home Affordable Modification Program under the Emergency Economic Stabilization Act of 2008, as amended, including, but not limited to, obligations relating to the modification of first lien mortgage loans and the provision of loan modification and foreclosure prevention services relating thereto.
DESCRIPTION OF PROGRAM SERVICES. The Grantee will provide:
a) Clean and sober transitional housing services at the Drake Hotel & Xxxx Hotel (SFAPD Clients)
b) Stabilization housing at the Broadway and Xxxxxx Hotels (SFCC)
c) Rental Subsidy Program: Includes, rental subsidy, move-in costs, and on a case by case basis, emergency rental assistance
d) Housing Application Workshops and Trainings

B. DCJESD agrees to continue to do the following:

2. Develop common confidentiality guidelines to share information between DCJESD and SRPD to the extent permitted by the California Education Code and the Welfare and Institutions Code governing client confidentiality.

DESCRIPTION OF PROGRAM SERVICES. A. YMCA’s Agreement: 1. The hours of operation will be from 6:30 AM – 6:00 PM, Monday through Friday, on days when school is in session.
DESCRIPTION OF PROGRAM SERVICES. The Company agrees to provide coaching and mentorship for the Program, including but not limited to the following: • The Program includes an online course with weekly email support during the first 12 weeks from the start date and options for additional coaching during and after the Program. The start date is defined as the date of signing the Agreement and where the program end date is defined as 12 weeks from that date. • Upon signing the Agreement and confirming upon start date, if the Client then wishes to defer the start of the Program to a later date, only deferred start dates of 30 days maximum from the original agreed-upon start date will be accepted and permitted. Deferrals longer than 30 days from the agreed-upon start date are not permitted. • After the Program is over, the Client may have the opportunity to extend coaching opportunities, where additional fees apply, and a different Agreement will be provided. • The Company has the right to modify the Program at any time for any reason and will ensure that the Client is provided with applicable updates if they are within the Program duration. • The Client can receive peer feedback and coaching support by posting questions in the online course (via the comments section) and attending additional coaching opportunities via Voxer, Zoom, or another communication system included at the time of the Program offer. All services required to be performed by a participating servicer relating to Treasury FHA-HAMP, as set forth in guidance issued by the Federal Housing Administration from time to time; including Mortgagee Letter 2009- 23, 2009-35, 2009-39, 2010-04 and 2010-10, and in the Program Documentation for including Treasury FHA- HAMP in the Home Affordable Modification Program under the Emergency Economic Stabilization Act of 2008, as amended, including, but not limited to, obligations relating to the modification of first lien mortgage loans insured by the Federal Housing Administration and the provision of loan modification and foreclosure prevention services relating thereto.

DESCRIPTION OF PROGRAM SERVICES. SPLA will provide volunteers to serve as one-‐to-‐one reading and writing tutors for students selected for the STEP UP program at the ARC campus. These tutors will meet at least once weekly for approximately two hours to provide remedial tutoring for students selected for the STEP UP program at the ARC campus. Tutoring will take place at the Learning Resource Center at the ARC campus or at a branch library.
