Dietary Services. If dietary services are provided, they shall be provided only as authorized by the hospice and in con- junction with the plan of care. The services shall be provided by a registered dietician or an individual who has documented equiv- alency in education or training. Dietary services shall be super- vised and evaluated by a registered dietician or other individual qualified under this paragraph who may delegate acts to other employees. Dietary services shall consist of:
1. Assessment of nutritional needs and food patterns;
2. Planning diets appropriate for meeting patient needs and preferences; and
3. Providing nutrition education and counseling to meet patient needs, as well as necessary consultation to hospice employees;
Dietary Services. A dietitian will meet with you to review your dietary needs and preferences soon after you arrive. The dietitian will also monitor your nutritional status throughout your stay and make recommendations as necessary.
Dietary Services. Contractor shall provide 1 FTE dietary aide staff on day and evening shifts, available to accommodate any special dietary needs of the Individuals, including serving meals in rooms as requested.
Dietary Services. Contractor shall provide .5 FTE for a Dietician position, dedicated to Individuals served under this contract. Person in this position will provide the following services:
a. Assess Individual nutritional needs;
b. Coordinate pre-admission person centered evaluation related to the Individual’s dietary habits, goals and preferences;
c. Work with facility RN or other involved medical professionals to evaluate appropriate diet concerns in relation to Individual health care needs;
d. Develop and implement Individual nutrition plans;
e. Nutritional plans should address health related concerns such as weight loss, strength-building, cholesterol, or diabetes management; and
f. Monitor dietary intake and results and adjust nutrition plans accordingly.
Dietary Services. 1. Local Government will provide a licensed, registered dietitian ("Dietician") to Facility to perform the following functions:
a. Provide input and, where necessary, approvals for inpatient nutrition care and related standards that affect the patient. This includes approval of menu nutritional adequacy and therapeutic compliance as well as the diet manual and policies and procedures related to nutrition care that are in place at Facility. Revised approvals of the Diet Manual from UMC El Paso will be provided to Facility when applicable or every three (3) fiscal years.
b. Provide nutrition screening and assessments on patients with development of individualized nutrition care plans to address the patient's medical nutrition therapy needs, to include supplemental nutrition, tube feedings, and nutrition education as determined necessary and as approved by the patient's medical staff member, with meals adjusted to patient preferences as needed.
c. Participate when requested in interdisciplinary meetings to discuss patient specifications and/or to provide input on policies, standards, or practices related to the Dietitian's expertise and scope of practice. Includes research by Xxxxxxxxx as needed into issues or practices, on and off Facility site, to include the Dietician's personal office or library.
d. Monitor the food service program for input on needed changes specific to a patient's individual needs or identifying trends to address on a larger scale through program or menu revisions.
e. Communicate among patients, Facility care givers, Facility Administration and the Food Service Management and/or contract administrators of the Contract. Provide follow-up on relevant issues.
f. Provide nutrition consults, assessments and education as ordered by the patient's physician. Consults will be completed within three (3) business days of receipt of the request for consult. Consult documentation must be completed in the Facility medical record (Clinician Workstation) within twenty-four (24) hours of the assessment being completed. The Nutritional Assessment will be reviewed and approved by the patient's attending medical staff member.
g. Complete a monthly report with HHSC-requested indicators agreed upon by the Parties and provide to the Pharmacy and Therapeutics Committee as well as to the Assistant Superintendent. Attend the meetings of Facility's Pharmacy and Therapeutics Committee.

Dietary Services. Alterra will furnish each Managed Facility, at Alterra's expense, with access to a Registered Dietician who will, under Alterra's supervision, advise and assist each Managed Facility in establishing dietary policies, philosophies, objectives and cost control methods to provide each Managed Facility's residents with dietary services. The Registered Dietician will advise and assist each Managed Facility in formulating, implementing and maintaining such policies, philosophies, objectives and cost control methods.
Dietary Services. Contractor shall provide a minimum ratio of 1 Dietary Services staff for every 7 Individuals served under the Contract, with a total of 6 FTE Dietary Services positions at full capacity. Staff in this position shall be on shift when the greatest number of Individuals are eating their meals and available to accommodate any special dietary needs of the residents, including serving meals in rooms as requested and assistance with eating.
Dietary Services. The Contractor shall have .5 FTE (minimum 80 hours a month) Dietitian services dedicated to the Individuals served by this Contract to perform the following duties.
(1) Assess individual nutritional needs;
(2) Coordinate pre-admission person centered evaluation on the Individual’s eating preferences;
(3) Assess Individual preferences and goals;
(4) Work with facility RN or other involved medical professionals to evaluate appropriate diet concerns in relation to individual health care needs;
(5) Develop and implement individual nutrition plans;
(6) Nutrition plans should address health related concerns such as weight loss, strength building, cholesterol or diabetes management;
(7) Monitor dietary intake and results and adjust nutrition plans accordingly; and
(8) Work with Individual and HealthCare Coordinator to form Support Network.
Dietary Services. Facility maintains a food service program monitored by a registered dietician. Regular meals, along with therapeutic diets and snacks prescribed by menu for each Resident, Facility will use reasonable efforts to recognize and accommodate individual preferences. Facility will provide a kosher meal for Resident upon request as long as it is understood that the Facility does not have a kosher kitchen on its premises.
Dietary Services. ALS will furnish the Facility, at ALS's expense, with access to a Registered Dietician who will, under ALS's supervision, advise and assist the Facility in establishing dietary policies, philosophies, objectives and cost control methods to provide the Facility's residents with dietary services. The Registered Dietician will advise and assist the Facility in formulating, implementing and maintaining such policies, philosophies, objectives and cost control methods.
