Disability Policy/Extended Illness Benefit. In recognition of the nurse’s need for income protection against long-term illness, a disability account has been established for each regular nurse. (During this CBA, the Hospital’s policies shall rename “Disability Leave” as the “Extended Illness Benefit”. At that time such new term shall be used by the parties under this Section, but it is understood that there shall be no changes to the rights and benefits under this Section.) Disability leave may be used to care for illness or injury of a dependent child under the age of eighteen (18) years. Disability Leave may also be used in the event of a health condition of a nurse’s child under the age of eighteen (18) that requires treatment or supervision, or for a child age eighteen (18) or older and incapable of self-care due to mental or physical disability, or in the event of a serious health condition or an emergency condition of a nurse’s spouse, domestic partner (as defined in Section 11.1, Health Insurance), son or daughter, parent, parent- in-law, or grandparent. All nurses, except for nurses who regularly work twelve (12) hour shifts, will earn Disability Leave at the rate of .02308 per each hour. Nurses who regularly work twelve (12) hour shifts will earn disability leave at the rate of .02564. Disability leave is earned for every hour in a paid status, including overtime and low census. Disability leave is not accrued on more than eighty (80) hours per pay period. Full-time nurses earn a maximum of six (6) days per year. Regular nurses will have the opportunity to accumulate an unlimited number of days in the disability account.   • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan. The plan shall provide post-probationary regular employees with salary continuation as per Appendix “A” until age sixty-five (65) in the event of a disability. The cost of the plan shall be borne by the Employer.

  • Life Insurance Benefits A. During the life of this Agreement, the basic life insurance benefit made available to Faculty members shall be calculated as 3 times base annual earnings, rounded to the next highest $1,000, but not more than $225,000. A separate additional benefit up to the amount of the life insurance will be paid for accidental death and dismemberment, or loss of sight. The amount of Life and Accidental Death and Dismemberment/Loss of Sight benefits will be reduced to 65% at age 65, and further reduced (from the original insurance amount) as follows: to 50% at age 70, and 35% at age 75. Basic life insurance and AD&D benefits will be provided with no employee contributions.

  • Long Term Disability Benefit In the event an employee, while covered under this plan, becomes totally disabled as a result of an accident or a sickness, then, after the employee has been totally disabled for seven (7) months, including periods approved in Section 1.3(a) and (c), he/she shall be eligible to receive a monthly benefit as follows:

  • DISABILITY INSURANCE PLAN Management shall expend for active employees of this unit who are members of LACERS the sum necessary to cover the cost of a basic disability insurance plan. Management shall also maintain a Supplemental Disability Insurance Plan, enrollment in which is at the discretion of each employee.   • Long Term Disability Benefits A benefit level of seventy percent (70%) of monthly earnings shall apply. Benefits would commence after a waiting period of seventeen (17) weeks, when Short Term Disability Benefits terminate. Terms of the Master Policy with the Insurance Company shall apply. Statement of Intent In order to go on LTD, the person must:

  • Disability Plan Effective midnight March 31, 1995, the College shall pay all of the costs of a Group Disability Insurance Plan which provides for a monthly taxable income of two-thirds (66- 2/3 %) of gross monthly salary. Administrative costs of this plan shall be borne by the College. Each full-time employee who has an appointment of four (4) months or longer, or continuing part-time employee with a workload of at least fifty percent (50%), is eligible to participate in the plan. Participation of eligible employees in this plan shall be a condition of employment. If an employee continues to be paid while awaiting receipt or adjudication of an LTD benefit such payment shall be considered an advance and will be repayable in full. Should full repayment not be immediately possible, Human Resources will agree on a repayment plan that is mutually acceptable to the College and the employee. The employee in receipt of Disability Income benefits under the group plan will have the opportunity to participate during a period not exceeding five (5) years in College benefit plans, as applicable but will be required to pay one hundred percent (100%) of the premium costs thereof. The employee in receipt of Disability Income benefits under the group plan will be entitled to return to the employ of the College during a period not exceeding five (5) years upon submission of a medical certificate indicating fitness for work. Should a single illness exceed ninety (90) calendar days in duration, an eligible employee must apply for disability insurance benefits in accordance with the terms of the plan. Seniority shall continue to accumulate during the period of disability to a maximum of five (5) years.

  • Long Term Disability Insurance 250. The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Long Term Disability Plan The Welfare Plan will include a Long Term Disability Plan summarized in Appendix “2”.

  • Short Term Disability Benefits Paragraph 1: The Board shall provide short term disability benefits as set forth in the Short Term Disability Summary Plan Description. Short term disa- bility benefits for disabilities resulting from non-occupational illness or injury, shall be paid at the rate of 70% of the teacher’s regular daily rate, subject to all applicable deductions. A teacher may choose to save up to five (5) accumulat- ed temporary leave days. Following the exhaustion of temporary leave, there is a five day waiting period before short term disability benefits begin. The five day waiting period will be waived for absences greater than 30 calendar days and short term disability payments shall be paid retroactively.

  • Supplementary Employment Insurance Benefits (1) Birth mothers who are entitled to maternity leave and who have applied for and are in receipt of Employment Insurance benefits are eligible to receive XXXX Plan payments.
