If management makes a change that affects a nurse’s paycheck, the nurse will be informed and a record of the change will be available to the nurse.

The Lessee and the Sublessee agree that, upon request of the Agency, the Lessee and the Sublessee shall furnish to the Agency such information as the Agency shall reasonably request as necessary to verify or confirm the information reported in Schedule A. Upon request by the Agency, the Lessee and the Sublessee shall submit to the Agency copies of each Form EEO-1 or the equivalent, with respect to the facilities of the Lessee and the Sublessee and their Affiliates within the Town, as are required to be prepared and filed with Federal or State authorities pursuant to applicable law. In addition, the Lessee and the Sublessee hereby authorize any private or governmental entity, including but not limited to The New York State Department of Labor (“DOL”), to release to the Agency and/or to the successors and assigns of either (collectively, the “Information Recipients”), any and all employment information under DOL’s control which is pertinent to the Lessee and the Sublessee and the Lessee’s and the Sublessee’s employees.
