Eye Care. A Member who seeks covered eye care may obtain such service directly from a participating ophthalmologist or optometrist who is licensed to provide eye care. Care is limited to medical conditions only, not routine vision care (except for children under age 19).
Eye Care. Employees are entitled to eye tests and equipment at MPI’s expense, to a maximum value of $500, every two years. Costs that are covered include: • eye examination; • frames; and • single vision, bi-focal or graduated lenses. In this clause, ‘lenses’ includes contact lenses. In addition, where for health and safety reasons, an employee requires an eye examination at more frequent intervals than every two years, MPI will meet the cost of that eye examination at the rate of up to $85 per examination.
Eye Care. All full and part-time employees shall be eligible to be covered on an annual basis by the NYSUT Group Benefits Vision Care Plan (“Designer Gold”).
Eye Care. At the Employee’s request, with the recommendation of his or her ophthalmologist, an Employee required to work on computers shall be given eye examinations every six months. Should such an examination not be covered by OHIP, the eye examination shall be paid for by the Museum.
Eye Care. Upon ratification up to $225.00 per family member, every two years and $75.00 dollars per family member to cover the cost of eye exams.
Eye Care. The City of Madison Heights shall pay the cost of a basic eye co-op program each year. Said cost shall be paid directly to the carrier but in no event to the employee.
Eye Care. Company will provide active employees with at least 90 days of service the NJ TRANSIT Bus Agreement Vision Plan.
Eye Care. Analyzers, Autorefractors, Auto Lensmeters, Auto Tonometers, Corneal Topographers, Cryosurgical Units, Electrosurgical Units, Lasers, Layout Blocking Systems, Ophthalmic Lens Blockers, Optical Biometry Systems, Patternless Edgers, Phacoemulsifier Aspirators, Refrigerators, Retinal Cameras, Slit Lamps, Surfacers, Surfacing Labs, Surgical Lights. Excluded Equipment
Eye Care. The E mp lo y e r will pay the cost of a n eye care program, the same as, or substantially similar to the one presently in effect for the term of this Agreement, for the employee or any member of the employee’s family.
1. Health Care Plan
Eye Care. 33.1 The Federation acknowledges that employees in certain circumstances may require eye testing in relation to their work with computers.
33.2 If glasses are prescribed for staff working specifically with computers and where no other reimbursement is available from a Health Fund, the Federation will pay for the glasses.
33.3 Where there is a charge for the eye test then the Federation will also consider reimbursement of this charge.
33.4 The Federation Health Society rates will be applicable for the reimbursement of the eye test charge and for the glasses prescribed.
33.5 Equipment installed will be in accordance with Australian Standards Association to ensure compliance with accepted tolerance levels.
33.6 The WorkCover standard is accepted as the monitor of radiation levels, glare, and reflection.
33.7 The Federation will be responsible for ensuring that the lighting and equipment meets the standard and needs for all employees.
33.8 All employees will be encouraged to take appropriate breaks from looking continuously at their computer screens.
