Hearing Care Sample Clauses

Hearing Care. The Colleges agree to pay seventy-five per cent (75%) of the premiums for the Hearing Aid Plan providing coverage to a maximum of three thousand dollars ($3,000.00) each three (3) years, per person, subject to eligibility requirements and enrolment requirements and the balance of the premiums shall be deducted by payroll deduction.

Hearing Care. The plan provides Benefits for routine hearing examinations for screening and for wearable Hearing Aids for covered Members. Coverage is limited to one routine screening exam for hearing-impairment and one hearing aid for each hearing-impaired ear every 36 months. Related items such as batteries, cords, and other assistive listening devices, including but not limited to, frequency modulation systems, are not covered. Hearing Care. CCS shall not be responsible for the provision of hearing services other than care for ear injuries or ear infections. CCS shall be responsible for providing any necessary replacement batteries for Inmates with hearing aid devices.

Hearing Care. Hearing Aids Modify the Plan to permit: • Two hearing aids (one per ear) every two years up to $3,000 maximum per aid; • Two hearing aid exams (one per ear) every two years shall be covered at 100%, in-network. Hearing aids must be Medically Necessary and a cost-effective model, as determined by the Claims Administrator. Fittings, repairs and adjustments are covered as needed. Effective January 1, 2020.
