Hospitalization, Surgical and Medical Insurance. Full-time teachers and those who teach at least one-half of the instructional day qualify for the hospitalization, surgical and medical insurance benefits currently in effect. Monthly employer contributions will be $503.18 for an employee only coverage, $948.41 for an employee + one coverage, and $1,144.60 for a family coverage. Teachers who were under contract prior to May 25, 2005 and who work less than one- half of the instructional day and who are being provided with full benefits will continue to receive full benefits for as long as they sign consecutive teaching contracts. If both spouses are full-time employees, only ONE spouse may elect coverage.

Hospitalization, Surgical and Medical Insurance. As of January 1, 2014 the school corporation’s contribution towards the total health plan may not exceed the State’s contribution of the state employee plans by more than twelve percent (12%). The plan benefits and carrier will not change without mutual agreement of the Board and Association. New teachers will be eligible for enrollment for thirty (30) calendar days from the first date of employment. Coverage shall begin the first of the month following the first contract work day. Teachers who wish to join the plan may do so within the first thirty (30) calendar days of the school year. Teachers may choose to join or drop at any time during the year if they have a family status change. Teachers’ yearly health plan coverage will be September 1 through August 31 so that teachers retiring, resigning, or taking an unpaid leave at the conclusion of a school year will remain on the plan through August 31. Retired teachers may choose to remain on the school corporation’s health insurance by paying the full premium until reaching age to qualify for medicare.

  • Hospital and Medical Insurance The University shall make available health insurance to the employees covered by this agreement to the same extent and in the same manner as is available to other University employees, such as Faculty and the Executive, Administrative and Professional Staff employees. It is the University's goal to have the same health insurance plans offered uniformly to all University groups and employees.

  • Medical Insurance Upon termination of employment, the Executive shall be entitled to all COBRA continuation benefits available under the Company's group health plans to similarly situated employees. To the extent permitted under Code Section 409A, during the applicable Payout Period, the Company shall provide such COBRA continuation benefits to the Executive at the active employee rates similarly situated employees must pay for such benefits. Upon the expiration of such Payout Period, the Executive will be responsible for paying the full COBRA premiums for the remaining COBRA continuation period.

  • Retiree Medical Insurance Retiree insurance coverage is included within each medical plan for all retirees under the age of 65 years, through self-payment. The Employer shall make available an appropriate medical plan for all eligible retirees ages 65 years or older.

  • Medical Care The Parents must comply with the School Welfare Officer's recommendations which may include a reasonable decision to release the Pupil home or to his / her education guardian when s/he is unwell.

  • Chiropractic Services This plan covers chiropractic visits up to the benefit limit shown in the Summary of Medical Benefits. The benefit limit applies to any visit for the purposes of chiropractic treatment or diagnosis.

  • Hospital Any institution which is legally licensed as a medical or surgical facility in the country in which it is located, which is a) primarily engaged in providing diagnostic and therapeutic facilities for clinical and surgical diagnosis, treatment and care of injured and sick persons by or under the supervision of a staff of physicians; and b) not a place of rest, a place for the aged or nursing or convalescent home or institution or a long term care facility.

  • Basic Medical Insurance All regular Employees may choose to be covered by the medical plan for which the British Columbia Medical Plan is the licensed carrier. Benefits and premiums shall be in accordance with the existing policy of the plan. The Employer will pay one hundred percent (100%) of the regular premium.

  • Medical There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.

  • Medical Care Leave An Employee who is unable to make the necessary arrangements for maintenance of personal health care outside of scheduled work time, shall be granted time off with pay. Such time off shall not exceed sixteen (16) working hours per calendar year. Hours in excess of sixteen (16) hours per calendar year shall be deducted from the Employee's sick leave accumulation.
