Life Insurance and Long-Term Disability. Employees will participate in the life insurance and long-term disability plan, in accordance with the terms applicable to the majority of the Hospital’s unrepresented employees. Employees may also participate in the voluntary life insurance plan offered by the Hospital.
Life Insurance and Long-Term Disability. During the term of this Agreement, 2 nurses will participate in the life insurance and long-term disability plan as in 3 accordance with the provisions of the SHS plan. During the term of this 4 Agreement, nurses may also participate in the voluntary insurance plans in 5 accordance with the provisions in the SHS plan.
Life Insurance and Long-Term Disability. The Executive shall continue to be eligible for life insurance and long-term disability benefits equals to those applicable to him on the Termination Date, subject to the same terms and conditions (including without limitation any provisions concerning payment of premiums) that generally apply to senior executives of the Company.

The Board shall make available a total of fifty thousand dollars ($50,000.00) term life insurance coverage with a total of fifty thousand dollars ($50,000.00) accidental death benefit. The Board shall pay all except twenty-six dollars ($26.00) of the annual cost of life insurance and long term disability.
Life Insurance and Long-Term Disability. The City agrees to pay the full amount of monthly cost for bargaining unit members who participate in the City’s group life and long-term disability insurance policy. Life Insurance coverage shall be in the amount of two times the employees' annual salary. Additional coverage may be purchased at the employee's expense.
Life Insurance and Long-Term Disability. For the term of the contract, the Board shall provide each permanent, regular (non-substitute) employee a fifty-thousand-dollar ($50,000) term life insurance policy and Long-term Disability coverage. Any reductions in coverage based on age shall be in accordance with ADEA.
Life Insurance and Long-Term Disability. To the extent permitted by the Company’s group insurance policies, the Company will cause to be continued Xxxxxx’x life insurance coverage and long term disability insurance coverage substantially equivalent to the coverage maintained by the Company for Xxxxxx prior to his separation at no premium cost to Xxxxxx. Such coverage will terminate upon the expiration of nineteen (19) months following the Separation Date. If such continuation is not permitted by the Company’s group insurance policies, the Company for a period of nineteen (19) months following the Separation Date will reimburse Xxxxxx for the premium cost incurred to obtain comparable life insurance and long term disability insurance under private policies, up to the amount the Company would have paid to provide such coverage to Xxxxxx had he remained an employee of the Company.
Life Insurance and Long-Term Disability. A. The City shall contribute the full cost towards a City sponsored long-term disability and group term life insurance program administered through Assurant.
B. Employees are responsible for the payment of any costs in excess of the maximum City contribution.
C. The City will provide Long-Term Disability insurance coverage equal to sixty-six and two-thirds percent (66.6%) salary per month subject to the maximum monthly benefit as described by the plan with a maximum ninety (90) calendar days wait period.
D. Group term life insurance coverage shall be $15,000 for natural death and $30,000 accidental death for employee only.
Life Insurance and Long-Term Disability. A. The City shall provide a group life insurance program providing for term life insurance equal to one and a half times (1 ½) the employee's annual salary, but in no case less than $25,000.
B. The City shall provide a Long Term Disability insurance program for full- time classified employees. This program shall include payment of sixty percent (60%) of an employee's monthly salary for a maximum monthly benefit of $5,000 after a 30-day waiting period. Disability insurance payment shall extend not beyond age 65 for disabilities caused by accident, and for a period not to exceed five years for disabilities caused by illness.
Life Insurance and Long-Term Disability. Employee shall be entitled to participate in all plans or agreements maintained by Employer for its executives and for employees for life insurance and long-term disability, subject to the terms and conditions of such plans in effect from time to time.
