Life Insurance and Long-Term Disability. Employees will participate in the life insurance and long-term disability plan, in accordance with the terms applicable to the majority of the Hospital’s unrepresented employees. Employees may also participate in the voluntary life insurance plan offered by the Hospital.

Life Insurance and Long-Term Disability. During the term of this Agreement, nurses will participate in the life insurance and long-term disability plan as in accordance with the provisions of the SHS plan. During the term of this Agreement, nurses may also participate in the voluntary insurance plans in accordance with the provisions in the SHS plan.

Life Insurance and Long-Term Disability. The Board agrees to adopt the plan of health insurance offered to school districts by the New York State Department of Civil Service in which premiums for individuals and dependent coverage under the Empire Plan Core Plus Enhancements will be paid in full by the District for all employees who work four (4) hours or more per day on a ten (10) or twelve (12) month schedule. Employees hired after July 1, 2006 who work at least four (4) but no more than five (5) hours per day, for 10 to 12 months, will be entitled to individual health benefits : not family coverage. Should the employee elect other available options, the District will contribute a dollar amount toward the payment of the premium equal to the amount that the District would pay had the employee elected the Empire Plan and the employee will contribute the balance of the premium. The Carrier/Broker to be mutually agreed upon. Further, on retirement only, after ten years of service in this District, the District will pay in full the cost of the Empire Plan Core Plus Enhancements premium for health insurance for the employee and dependents. Further, on retirement only, after five (5) years of service in this District, the District will pay in full the cost of the Empire Plan Core Plus Enhancements premiums for health insurance for the employee. Employees hired after July 1 ,2 0 0 6 , in retirement will be required to have ten (10) years of service in the District to be eligible for health insurance coverage for the employee and/or dependents. The District will pay in full the cost of the Empire Plan Core Plus Enhancements premium for health insurance for the employee and dependents, minus the employee's contribution rate the day before retirement.
