They would like to formally request access to the EAG workplan to understand how this is being prioritised. The Chair of the EAG stated that the workplan had been discussed at the 12 February 2019 meeting, but as the minutes of the NAAR meeting were not available at the time, the group were not able to take this into consideration. However, the Chair will ensure that this item is on the agenda for the EAG’s next meeting on 16 April 2019. Action Description Responsible

Long Term Conditions. A Long Term Condition (LTC) is one that cannot currently be cured but can be controlled with the use of medication and/or other therapies1. The most prevalent LTCs2 in Wandsworth are set out in Figure 1 below:

Long Term Conditions. (LTC) (Alchemy) Removal of LTC caps in different localities Te Whatu Ora Nine of the twenty districts do not have LTC caps. Te Whatu Ora has inherited variation across districts in a number of areas, including LTC, and this needs to be worked through. Where a national approach can be applied this will be done in time. Does LTC need to be considered as part of the EAG &/or uplift conversations? CCPG The LTC service needs to be reviewed to ensure it meets the needs of patients and improves outcomes for them. Reviews were paused until April 2023 and a further pause needs to be considered as they involve a lot of paperwork Action Green Cross Health The LTC service is very good for patients, pharmacy does a really good job with the service, but having caps restricts access to the service. Alchemy Group The elimination of LTC caps needs to be expedited. Would be interested in being involved in any service review. Te Whatu Ora Will consider whether a review of the LTC service and its funding (possibly within an EAG) and elimination of districts caps can be progressed ahead of new agreement.
