Ongoing Eligibility. A member's eligibility for coverage under this contract will cease on the earlier of:

Ongoing Eligibility. The ongoing eligibility period is from September 1, through August 31, of each year. Extra Help workers in Category A are not required to work 30 hours per pay period during the “off-season”. At the end of the working season, when Category A individuals drop off payroll they will be notified that they can elect to continue coverage or drop coverage. If they choose to continue coverage, they pay 100% of the premium. If they fail to pay the premium during any pay period they will be terminated from coverage, offered COBRA, and they will not be eligible for coverage again until they are re-hired. Such coverage will begin the first of the month following their re-employment. If Category A extra-help workers choose to drop coverage, they can re-enroll if they are re-hired for the next season and are once again in a paid status. Coverage will begin the first of the month following their re- enrollment. At the beginning of each fiscal year, extra-help workers who have worked 750 hours in the prior fiscal year will be eligible for the following year’s coverage. Any individual who is not eligible (after having been eligible in the prior year) will be offered COBRA. Health coverage will be terminated for covered extra-help workers who:
1. Terminate employment during the season (benefits terminate at the end of the month of termination).
2. Fail to work 750 hours, as determined the following July (benefits terminate effective August 31, and COBRA is offered).
3. Fail to work 30 hours per pay period in 2 consecutive pay periods during the season of employment (benefits terminate effective the end of the month of termination).
4. Fail to pay within a single pay period their portion of cost (during the season or if they have elected coverage in the off-season, during the off-season). Example: ▇▇▇▇ is a Park Aide who normally works from April through October and has more than 750 hours in 2005-2006. She elects to enroll during the August open enrollment and receives health coverage effective September 1. ▇▇▇▇ continues to work until the end of October and is terminated because the season is over. ▇▇▇▇ is given the chance to continue coverage until next April, at her own cost. If she elects coverage, she will be covered during the off-season, provided that she pays 100% of the premium on a timely (pay period) basis. Ongoing Eligibility. To receive a contribution to a health insurance for a month, an employee must be in a pay status for at least eight (8) hours in the prior month. Effective January 1, 2007, an employee will be taken off this benefit effective July 1, if an employee is in a pay-status for an average of less-than twenty (20) hours a week during January, February, and March. Management agrees not to reduce work hours of such employees for the sole purpose of denying them this benefit.

Ongoing Eligibility. Ongoing eligibility is determined by the WHC in consultation with the Transitional Plus Support Provider and is dependent upon the tenant’s continuing need for housing and support and the tenant meeting the agreed commitments in their support plan. Tenants who have met their tenancy obligations and who maintain eligibility for the THP-DV program, may have their lease renewed for a further six (6) month fixed term, as long as the period of continuous tenure does not exceed five (5) years – the maximum allowable for THP-DV.
Ongoing Eligibility. Ongoing eligibility assessments are conducted three months before the end of each fixed term. Tenants must provide evidence of continued eligibility, to namely satisfy: • the general eligibility criteria, demonstrate an ongoing need for subsidised housing and not have assets which could reasonably be expected to meet their housing need; and • the Women’s Housing Company criteria; and • a tenant’s household income may be up to 25% above the maximum initial eligibility limit for the moderate income band before becoming ineligible. Tenants agree to supply their income details to the WHC under section 187 of the NSW Residential Tenancies Act 2010. To determine gross assessable income, tenants must provide income and assets evidence in accordance with the Rent Policy.
Ongoing Eligibility. The Credit Union may close your Account if you are no longer a member in good standing. A member in good standing must: • Have brought a negative balance in your checking account to a positive balance within 30 days of the overdraft occurrence; • Not be delinquent on a Credit Union loan (defined as more than 30 days past due); • Not have a modified loan with the Credit Union; and • Not be the subject of any legal or administrative order or levy on a Credit Union account; Failure to meet these eligibility requirements will result in the Credit Union revoking your account.
Ongoing Eligibility. It is the Employer’s intent that employees enrolled in the benefits plans are to remain enrolled for a minimum one (1) year period.
Ongoing Eligibility. Once you have opted in to Courtesy Coverage, the Courtesy Coverage feature may continue to be available for you as long as you continue to meet the eligibility criteria in each period of 35 days after initial activation and your Spending Account remains in good standing. However, we may choose to consider additional sources of deposits other than ACH credits, in our sole discretion, to determine whether you meet the requirement to have at least $200 in qualifying deposits in the relevant 35-day period.
