Benefits Eligibility. All employees who are regularly scheduled to work at least twenty (20) hours per week are eligible to participate in the health insurance benefit program offered by the Employer to a majority of its employees who are not in a bargaining unit. Employees shall be offered benefit options, in accordance with the terms of the Employer’s program, with regard to medical, dental, vision, life, AD&D, long-term disability and short-term disability plans, and healthcare and dependent care spending accounts.

Benefits Eligibility. This clause applies only to employees on a full-time continuing appointment, 50% part-time continuing employees who hold six-month appointments and, with the exception of section 39.1.3, to employees on full-time term appointments which are greater than five calendar months in length. OC shall pay the full premiums for the health and welfare plans while the employee is in receipt of a salary from OC.

Preschool and kindergarten teachers receiving full time insurance benefits shall continue to receive full time benefits. These part time teachers shall receive health insurance and dental insurance with the Board paying 85% of the monthly premiums as stated in this article.
Benefits Eligibility. Unless otherwise provided, probationary employees shall not be entitled to fringe benefits set forth in this Agreement during their period of probationary employment and there shall be no retroactive payment for the same upon the successful completion of such period. Such probationary employees, however, shall be paid the contractual minimum wage rate for the classification in which they are placed. In those areas where Health, Welfare and Pension funds require payment then Employer shall make the necessary contributions.

Regular full-time employees shall be eligible for medical, dental, vision and life insurance coverage the first of the month following thirty (30) days of eligible service. Medical, dental, vision and life insurance benefits will be paid at 100% of the City contribution for those employees who have a Standard Hours designation of at least seventy-two (72) hours in a pay period in a benefits eligible, budgeted position. 3. Regular part-time employees will be eligible for medical, dental, vision and life insurance coverage the first of the month following 174 hours of eligible service. Medical, dental, vision and life insurance benefits will be paid at 50% of the City contribution for any regular employee who has a Standard Hours designation of at least forty (40) hours but less than seventy-two (72) hours in a pay period in a benefits eligible, budgeted position. 4. Medical, dental, vision and life insurance benefits may be denied to employees who are in a pay status for less than eighty (80) hours during a calendar month by the withholding of City-paid premiums for the subsequent month.
Benefits Eligibility. Part-Time Employees A. Group I: Eligible Group I part-time employees shall pay 25% of the total premium cost for single and family coverage for the medical, dental and basic vision plan coverage in which they enroll. Eligible Group I part-time employees who elect to waive health insurance coverage and elect the taxable cash waiver option. Such employees shall receive an annual payment of up to $900.00 {pro-rated to the time of election within the plan year} subject to the provisions and limitations of Article 17 applicable to the full-time employee taxable cash waiver option. Effective July 1, 2016, the taxable cash waiver option shall not be available to employees who are not then participants. For the term of this agreement, employees receiving the taxable cash waiver option payment as of July 1, 2016 will be grandfathered and will continue to be eligible for the waiver payment, while waiving coverage. Part-time employees who choose to enroll in the Opt-up vision plan will pay the difference between the amount the University pays for part-time employees for the basic vision plan and the total premium cost of the Opt-up vision plan. The University shall pay 100% of the premium cost for basic life, basic accidental death and dismemberment and long term disability coverage for these part-time employees. In addition, Group I part-time employees may also enroll in the following benefits plans: 1. Medical Flexible Spending Account 2. Dependent Daycare Flexible Spending Account Plan 3. Voluntary Group Life Insurance Group I part-time employees who choose to enroll in these plans will pay the full cost for their participation. B. Group II: Group II part-time employees may enroll in the following benefit plans: 1. Single and Family Health Care Coverage (Exclusive Provider Option Plan only) if and when available 2. Medical Flexible Spending Account 3. Dependent Daycare Flexible Spending Account Plan 4. Voluntary Group Life Insurance for the employee only up to $150,000 maximum Group II part-time employees who choose to enroll in these plans will pay the full cost for their participation.

Medical, dental, vision and life insurance benefits will be paid at 100% of the city contribution for those employees who have a Standard Hours designation of at least seventy-two hours in a pay period in a benefits eligible, budgeted position. 12.2.2 Following an authorized unpaid leave, a permanent full- time employee shall be eligible for medical, dental, vision, and life insurance coverage on the first calendar day of the month in which the employee returned to active employment. 12.2.3 Permanent part-time employees will be eligible for medical, dental, vision and life insurance coverage the first of the month following the date of hire. Permanent part-time employees shall cease to be eligible for as provided herein for medical, dental, vision and life insurance coverage on the last day of the month following the date of unpaid leave status or their separation from active employment. The amount of contributions which the City will make on behalf of permanent part-time employees for medical, dental, vision and life insurance benefits shall be as follows: Standard Hours Per Pay Period Percentage of Full-Time Employee Contribution 12.2.4 The percentage of benefits shall be based on the employee’s standard hour designation for the period of part-time (or job-share) work under the program. 12.2.5 Medical, dental, vision and life insurance benefits may be denied to employees who are in a pay status for less than seventy-six (76) hours during a calendar month by the withholding of city-paid premiums for the subsequent month.
