Plans Offered. The College will offer to all Classified members the following healthcare plans:
1. Moda Plan 2 Connexus w/ Pharmacy
2. Moda Plan 6 Connexus (HSA) w/ Pharmacy
3. Kaiser 1 w/ Pharmacy
4. Kaiser 3 w/ Pharmacy
Plans Offered. The School agrees to provide retirement benefits through the New York State Employees' Retirement System to Non-Teaching Service Association members who have elected to join the System. On July 1,1987 the School agrees to adopt the following benefit plans of the System:
18.1.1 Section 75-1 (including 75-e, 75-g), the New Career Plan for Tier 1 and Tier 2 Non-teaching Service Association Members.
18.1.2 Section 41-j, The Allowance for Unused Sick Leave, for all Non-Teaching Service Association Members who have elected to participate in the New York State Employees Retirement System.
18.1.3 Sections 5G1 (4) and 2854 (3)(c), as applicable, for teaching assistants who are members of the Teachers' Retirement System.
Plans Offered. The District shall provide administrators regularly working at least thirty (30) hours each week with health care coverage equivalent to that provided under:
1. Healthy Blue High Deductible "50 Plan": Effective January 1, 2014, the parties agree that all eligible unit members are to enroll in the health care plan of the FLASHP called Healthy Blue High Deductible "50 Plan" with a prescription drug plan with copays of $5 generic drugs/$35 preferred brand name drugs and $70 copay [after meeting required deductible] for non-preferred drugs.
Plans Offered. The City currently provides full family medical through City County Insurance Services (CIS). Blue Cross/Blue Shield (BCBS) Copay A Rx4 Plan; VSP Vision; Delta Dental Plan III with Ortho; and Alternative Care Rider- Chiropractic and Acupuncture to eligible employees as defined by Federal and/or State laws, including but not limited to the Affordable Care Act (ACA). Employee contribution toward the monthly health care premium is through payroll deduction. A representative from the local union will participate in the annual benefit committee to review plans offered each year.

Plans Offered. The Employer participates in the Golden LivingCenter Health Insurance and other Welfare plans. At the present time, the plans include Golden LivingCenter Medical Plans, Health Care Spending Account, Dependent Care Assistance Program, Tuition Reimbursement, Dental, Vision, and Xxxxxxx Enterprises, Inc. 401(k) Savings Plus Plan. The specific benefits of these plans are occasionally changed, modified, or improved, including the amount paid for coverage for such plan by the Employer and employees who elect coverage. In the event such changes, modifications, or improvements occur during the life of this Agreement, the Employer need not seek the Union’s prior agreement, but the Employer will promptly notify the Union of the changes or improvements and the effective date thereof.
Plans Offered. The City will maintain its contract with PERS to provide coverage under the Public Employees' Medical and Hospital Care Act. Annuitants may participate at their cost subject to PERS regulations.
Plans Offered. The City shall provide full family medical, dental and vision coverage for the life of this Agreement through CIS regence Copay Plan E RX7 and CIS Delta Dental II, if available or a substantially comparable plan. The alternative plan available to employees is the Xxxxxx Copay Plan B with prescription, vision, and dental coverage. Willamette Dental Plan A is also available.
Plans Offered. The City will contract with Fire Districts Association of California – Employment Benefits Authority to provide coverage under a Joint Powers Authority Agreement (JPA). Annuitants may participate at their cost subject to regulations.
