Procedures for Individualized Notice. SSA will provide direct notice, in writing, to all applicants or representative payees, at the time of application for Medicare benefits that SSA will match applicant records against those of other agencies to verify their eligibility. SSA will periodically provide similar notices to all recipients at least once during the life of the match. SSA will also publish notice of this matching program in the Federal Register, in accordance with the requirements of the Privacy Act and OMB guidelines. OPM will provide notice on its application forms or in a separate handout when necessary. OPM will provide subsequent notices, via yearly mailings, to its respective retirees, annuitants, beneficiaries, recipients, and their representatives as required, describing OPM’s matching activities.

RRB's and SSA's notices consist of appropriate language printed on their application forms or a separate handout with appropriate language when necessary.
B. Beneficiaries/Annuitants RRB and SSA will provide subsequent notices to their respective retirees, annuitants, beneficiaries, or recipients as required. RRB's and SSA's notice will consist of a notice of this matching program in the Fed. Reg. followed by periodic mailings by each agency to the retirees, annuitants, beneficiaries, and recipients under the programs administered by the respective agencies. The periodic notices will include a description of the matching activities of the respective agencies. Separate appendices to this agreement describe the procedures for verification and individualized notice affording an opportunity to contest match results.

Procedures for Individualized Notice. A. HUD HUD, through its POAs, will notify all applicants who apply for, and participants who are receiving, housing assistance that HUD will conduct computer matching programs. Participants in HUD rental assistance housing programs are notified that HUD and the POAs are requesting income and other information to verify household income, identity, and death information through computer matching programs to ensure program eligibility for assisted housing benefits, and to ensure that the level of benefits are set correctly. POAs will provide applicants and participants with a consent form developed by HUD, or the POA, in accordance with 24 C.F.R. § 5.230. Individual applicants and participants will sign the consent form when applying for initial or continued assistance. XXX published a final rule concerning the participant's consent to release of information on March 20, 1995 (60 Fed. Reg. 14632). Each adult member of the household receives an individual notice at initial certification (or examination) and at each recertification (or reexamination). Each adult member of the household must sign an Authorization for the Release of Information (Form HUD-9886 or Form HUD-9887) or an equivalent consent form that meets the criteria specified at 24 C.F.R. § 5.230. HUD may deny housing assistance if an adult household member fails to sign an authorization form. The forms incorporate the requirements of 42 U.S.C. § 3544(b) and provide applicants and/or participants with a Privacy Act notice. The Privacy Act notice states that HUD, through POAs, may use the information applicants and/or participants provide on their application for housing assistance in a computer match to verify information the applicants and/or participants provided.
