Housing Assistance. The Employer agrees to provide, upon application by an employee, a salary advance for the purpose of buying, for personal use, a principal residence in the place of work assigned by the employer or its environs. This salary advance shall not exceed $30,000 and shall be made for a maximum period of five years. In the event of termination of an employee's employment prior to expiry of the period of repayment of the advance, the outstanding balance owed by the employee shall be repaid immediately. The recipient of the advance who wishes to repay it earlier may do so. The advance shall be secured by a promissory note drawn in favour of the Employer. There shall only be one advance issued per employee at any one time. Failure to comply with the terms of the advance or cessation of employment at the University renders the advance repayable at the discretion of the Employer.

Housing Assistance. (i) Where the employee owns a home, financial assistance will be provided to cover the actual and reasonable costs of: (1) advertising the sale of the home if sold privately or real estate agents fees if sold by a real estate firm, (2) legal fees, (3) mortgage repayment penalty, if applicable. (ii) Where the employee has been renting accommodation, actual and reasonable costs required to cover termination of a lease. (iii) Where an employee purchases a home in the new geographic location, payment of legal fees pertaining to good title of property and acquisition of mortgage.
Housing Assistance. (2.2, 2.3, 2.4, 2.5 AND 2.6): These services are necessary to ensure the health, welfare, and safety of the client in his or her physical residence or home setting. As specified in the client’s plan of care, services may include provision of physical adaptations and assistive devices, emergency assistance in situations which demand relocation and assistance to restore utility service. Housing Assistance services include: a. MINOR HOME REPAIRS AND MAINTENANCE (2.2): Minor Home Repairs do not involve major structural changes or repairs to a dwelling. Maintenance is defined as those services necessary for accessibility (e.g., ramps, grab bars, handrails, items above what is covered by the State Plan, and installation), safety (e.g., electrical wiring, smoke alarms), or security (e.g., locks). Eligible clients are those whose health and/or safety or independence are jeopardized because of deficiencies in their place of residence. This service is limited to clients who are owners/occupiers of their own home, or those in rental housing where the owner refuses to make needed repairs or otherwise alter the residence to adapt to special client needs. Written permission from the landlord (including provision for removal of modifications, if necessary) is required before undertaking repairs or maintenance on leased premises. All services shall be provided in accordance with applicable State or local building codes. b. NONMEDICAL HOME EQUIPMENT (2.3): Includes those assistive devices, appliances and supplies which are necessary to assure the client’s health, safety and independence. This service includes the purchase or repair of nonmedical home equipment and appliances such as refrigerators, stoves, microwave ovens, blenders, kitchenware, heaters, air conditioners, fans, washing machines, dryers, vacuum cleaners, furniture (i.e., couches, lamps, tables, chairs mattresses, bedding, and emergency supply kits and goods) under the following circumstances: i. The client is receiving Deinstitutional Care Management services, and the items are required to facilitate discharge from the institution to a community residence ii. The client’s assessment identifies the need for this service including how it is a necessary support if the client is to remain in the community, and the care plan specifies the required item(s) iii. Housing Assistance. Contractor shall provide HDAP eligible participants with housing assistance, including but not limited to interim housing, recuperative care, congregate care, landlord incentives, navigation services, individual housing support payments, rental subsidies, housing identification processes and support. The three components of housing assistance as defined by the HDAP are: 1. Housing assistance – financial assistance related to housing that is provided in coordination with both housing navigation and housing specific case management services and shall be provided to participants concurrently throughout the disability application process.
Housing Assistance. The Parties contemplate that, starting on the Effective Date, the city will provide assistance to Engeland for the rental or lease of housing in Los Altos through a cash payment to be dispersed to Engeland in equal payments monthly. The City will bear the actual and reasonable monthly rental or lease cost in the amount of Two Thousand Five Hundred ($2,500) Dollars per month, which shall not exceed a total annual amount of Thirty Thousand ($30,000) Dollars. CONTRACTOR’S Housing Navigators shall work with participants in identifying housing locations and applying for housing options.

CONTRACTOR shall use PROGRAM funds to provide a variety of temporary and interim housing options based on the needs and preferences of eligible participants. Housing options include: A. Interim shelter options, including shelters, motels, bridge housing, or any other temporary shelter placements not intended or considered to be permanent housing B. Recuperative care and recuperative care services C. Move in costs and housing stabilization costs including security, utility, and pet deposits, storage fees, moving costs, and costs associated with making a home habitable D. Modifications to units in order to accommodate accessibility needs, i.e. making units compliant under the Americans with Disabilities Act (ADA) E. Independent Living Facilities, recovery residences, and board and care facility placements, as long as placements are compliant with Housing First F. Reunification with family or friends or other shared housing opportunities

Services must be offered in a manner consistent with Housing First and actively employ evidence-based practices for participant engagement. g. CONTRACTOR’S Housing Case Managers shall provide continued case management to participants placed in interim housing with HDAP funds to ensure transition to permanent housing options and completion of securing all identified income sources.
