REPORTS BY CONTRACTOR. In addition to any reports required elsewhere under this Contract, including Exhibit A (Statement of Work) and Attachment A.1 (Tasks and Deliverables) to this Contract, in order to control expenditures and to ensure the reporting of all Work provided by Contractor, Contractor must provide to County Project Manager, as frequently as may be requested by County Project Manager, but in no event more frequently than weekly, written reports which must include, at a minimum, the following information: a. Period covered by the report, b. Summary of Project status as of reporting xxxx, x. Overview of the Work provided during the reporting period, d. Progress status of each Work component scheduled for the reporting period, e. Issues/problems encountered, proposed resolutions and projected completion dates for problem resolution, f. Status of contractually defined Deliverables, milestones and walk-throughs scheduled in the Project Schedule, g. Action items and decisions from the previous meeting, h. Planned activities for the next two reporting periods, and i. Any other information which the County may from time-to-time require.

REPORTS BY CONTRACTOR. Each year, 90 days before the anniversary date of this contract, Contractor must furnish a report of the total services ordered under this contract during the preceding 12 months. The report must be in a format acceptable to City and must list by department or location: all services awarded under this contract; total quantity and dollar value of each service ordered, including services for which there were no orders. Contractor must also furnish a separate similar report for the total of all services ordered by City which are not part of this contract. The Contractor shall, at any time during the term of this Contract, upon written request of the Owner, render a report of inspections, repairs or replacements made by the Contractor at the premises herein, itemized as to parts installed or services performed and supply samples of lubricants, compounds, or other materials employed. 1. Contractor shall prepare and issue all required forms and/or reports relative to examinations, tests, and inspections as specified herein.

REPORTS BY CONTRACTOR. The Contractor shall quarterly (within thirty [30] days after the quarter ends) or at any time during the term of this Contract, upon written request of the Owner’s Representative (within thirty [30] days of the request), render a report of maintenance inspections, callbacks, repairs or replacements made by the Contractor at the premises herein, itemized as to building, unit number, maintenance hours provided, parts installed, services performed and supply samples of lubricants, compounds, or other materials employed if requested. Vendor’s maintenance/record keeping system shall comply with the requirements of the applicable ASME a17.1 maintenance control program (MCP) and Minnesota building code rule 1307 (Minnesota elevator code). 1. Quarterly reporting requirements shall include a quarterly summary of maintenance hours detailed by name of employee, date work was performed and location of work by building name. In addition, the quarterly report shall include completed versus scheduled preventative maintenance procedures and planned repairs for the just completed quarter and a twelve (12) week projected schedule for preventative maintenance procedures and planned repairs. 2. Quarterly reporting requirements shall include total maintenance hours with a breakdown indicating the number of hours assigned, by building, for preventive maintenance, regular time callbacks, overtime callbacks and repairs. Backup documentation for the individual categories will be provided as requested by the Owner. 3. Quarterly reporting requirements shall include updated Safety Test Matrix’s on a per building/per unit basis. 4. All quarterly contractor reports shall be uploaded into the current MPHA cloud-based website.
REPORTS BY CONTRACTOR. Contractor will submit inspection reports weekly to University’s Facilities Manager or Designated Representative. Inspection reports will be prepared and signed by Xxxxxxxxxx's On-Site Manager or Shift Supervisor. At University’s request, Contractor will at any time during the term of this Agreement provide a report on inspections, maintenance schedules, equipment, staffing, emergencies, security problems or any related matters in connection with the Service Areas or other University premises. In addition, Contractor will submit one (1) complete sets of all Material Safety Data Sheets (“MSDS”) to University’s Facilities Manager or Designated Representative in advance for all materials being used by Contractor in the Service Areas or on other University premises.

(b) Overview of the reporting period. (c) Tasks, subtasks, deliverables, goods, services and other work scheduled for the reporting period which were not completed. (d) Tasks, subtasks, deliverables, goods, services and other work for the reporting period which were completed. (e) Tasks, subtasks, deliverables, goods, services and other work completed in the reporting period which were not scheduled. (f) Tasks, subtasks, deliverables, goods, services and other work to be completed in the next reporting period. (g) Issues to be resolved. (h) Issues resolved. (i) Summary of project status as of reporting date. (j) Updated milestone chart. (k) Any other information that COUNTY may from time-to-time require. 22. Subparagraph 4.4.1 (General) of the body of the Agreement is amended to read as follows:

The Contractor shall cooperate with the Township as necessary to review financial statements and/or audit Contractor’s books and records concerning this Agreement.
