Requirements for Participation. (a) Any person who becomes an eligible employee of the City shall be required to participate in the Retirement Fund; provided no person who becomes an eligible employee on or after his sixtieth (60) birthday may participate in the Retirement Fund.

Students who wish to participate in the Exchange Program will be screened for eligibility for admission by the Home University, which shall respect the admission requirements and enrollment constraints of the Host University. Students will be required to comply with the standard rules, regulations, and enrollment restrictions of the Host University in the selection of courses, including requirements of language proficiency and health documentation. Students must be proficient in the language of instruction for courses being taken at the Host University in order to participate. At Georgia State, the language of instruction is English, and at International Institution the language of instruction is language. Students from International Institution will be required to submit English language proficiency test scores appropriate for admission: List required tests and respective scores. At the discretion of the Program Director, students from International Institution may submit the Georgia State English Language Proficiency Form in place of TOEFL scores. Students from Georgia State will/will not be required to demonstrate proficiency in language by List language requirements. These language requirements apply only to the students and not to their spouses and/or dependents. Each Host University shall have final authority over decisions regarding admission of students to its courses.

The requirements and compensation for the Senior Firefighter Program are set at two levels and are summarized below: Senior Firefighter I Eight years paid experience (at least five with Vacaville) California State Fire Marshal Firefighter II certification Special assignment(s) working a 40-hour work week schedule for a cumulative total of at least 6 months (in Vacaville, not including “light duty” assignments) or involvement in special project(s) or program(s) approved by the Department for a total of at least 12 months (in Vacaville). Credit for previous assignment will be granted from September 16, 2000, forward. Senior Firefighter II Twelve years paid experience (at least nine with Vacaville) Certification in any California State Fire Marshal specialty track or CSTI hazardous materials technician or specialist or certified confined space operations or certified rescue systems 1 or 2 California State Fire Marshal Fire Officer certificates and education courses. Special assignment(s) working a 40-hour work week schedule for a cumulative total of at least 12 months (in Vacaville, not including “light duty” assignments) or involvement in special project(s) or program(s) approved by the Department for a total of at least 24 months (in Vacaville). Credit for previous assignment will be granted from September 16, 2000, forward.

The Participating Employer agrees that it will: 🞎 Provide the Plan Administrator at Church Extension Plan, or its appointee, with any information or documentation necessary or desirable for Plan administration or legal compliance 🞎 Furnish its employees with any notices, election forms, or other materials provided to the Participating Employer for distribution to its employees 🞎 Pay its proportionate share of plan expenses as assessed by the Plan Administrator 🞎 Submit a written notice to the Plan Administrator at Church Extension Plan when an employee has terminated their employment Continued on next page > TERM S AND CONDITIONS OF PARTICIPATION (CONTINUED) 3 Withdrawal from Participation 🞎 The Participating Employer may withdraw from participation in the plan at any time by giving written notice to its employees and to Church Extension Plan, the 403(b) “Trustee” and the 403(b) Plan Administrator. 🞎 Any distribution or transfer of employees’ account balances held by the plan, whether to the Participating Employer’s employees or to a new plan, will be net of the Participating Employer’s proportionate share of any plan administrative, maintenance, and investment management expenses that are properly chargeable to the Participating Employer and are unpaid as of the withdrawal date, unless those expenses are paid directly by the Participating Employer on or before the withdrawal date.
Requirements for Participation. 2.1 To participate in the South Bay Region Node, Member Agencies agree to share costs based on the “member population” computation as described in Section 4 below. Other cost sharing formulas may be proposed and considered by the South Bay Region Node Board of Directors when the MOU is renewed or amended.
Requirements for Participation. The PBM shall require each Network Pharmacy to enter into agreements consistent with applicable laws and CMS participation requirements. In pertinent part, but not limited to, PBM shall ensure that Network Pharmacy Agreements include a provision that: • If and when such CMS regulatory requirement goes into effect, requires PBM to issue, mail or transmit payment to Pharmacies within fourteen (14) days of PBM’s receipt of an electronic claim, and within thirty (30) days of PBM’s receipt of a non-electronic claim; and • To the extent PBM utilizes an external pricing source to calculate reimbursement to Network Pharmacies (such as AWP), PBM shall explicitly identify such source in Network Pharmacy Agreements. In addition, PBM shall also add a provision to such Network Pharmacy Agreements indicating that updates to such pricing source shall occur on January 1st of each year, and thereafter no less frequently than once every seven (7) days.
Requirements for Participation a. Tenured with exceeds or meets evaluations in all areas for at least five (5) years including at least two (2) years in this school district. Teachers can participate in this program in lieu of a formal evaluation providing they have satisfactory ratings within the last two evaluation periods and with site administrator approval of the project.
Requirements for Participation. The Leadership Incentive Program is composed of two incentive levels for Sergeants, two incentive levels for Lieutenants, and two levels for Captain. The levels of compensation are independent of one another and are not cumulative.
