Shelter Services. In consideration of the payments set forth in Exhibit “B”, Contractor shall provide the following shelter services: The InnVision Facility at which services are to be performed is located at 000 Xxxxxx Xxxx, Xxxx., 000-X, Xxxxx Xxxx, XX 00000 (hereinafter referred to as “premises” or “facility”). The facility serves as a multiple purpose shelter/transitional housing facility and day service center for homeless families and individuals. Contractor shall provide services in connection with those multiple purposes as set forth herein. Contractor shall at all times operate the facility in accordance with the applicable federal and state rules and regulations as well as all government safety, health and sanitation standards.
Shelter Services. Shelter shall adhere to the scope of work described in “Exhibit A”, attached hereto and by this reference made a part hereof (collectively the “Services”). University shall reimburse Shelter for costs (“Expenses”) pursuant to Section 2 below and as outlined in Exhibit
Shelter Services. City shall provide the shelter services (“Shelter Services”) to the residents of National City in the manner and type described below:
Shelter Services. IHS shelter services shall be open to the public seven (7) days per week from 10:00 a.m. to 6:00 p.m. For purposes of this Agreement, “Shelter Services” shall include:
Shelter Services i) Maintain, or cause to be maintained, an animal shelter whereat stray, owner surrendered, confiscated, quarantined and custodially impounded household pets are received and provided with appropriate care, housing, and shelter services in accordance with state regulations, local ordinances, and policies governing the humane treatment of such animals.
ii) Provide services allowing sheltered animals to be returned to their owners, adopted into new homes, or to be humanely euthanized.
iii) Provide for the receipt and publication of lost and found animal reports.
iv) Provide services for the humane euthanasia of household pets and for the disposal of their remains.
Shelter Services. At nighttime, the CONTRACTOR shall operate the PROGRAM area as 32 an overnight shelter which will provide safe sleep for up to 60 women seeking a safe place to rest, three 33 meals a day, and access to hygiene services, showers and restrooms.
Shelter Services. Contractor shall provide temporary living situations while the individual and program staff are locating more appropriate housing. The contractor shall insure that these temporary living situations are safe and meet minimal housing standards. The contractor shall strive to limit the use of shelters to a minimum and whenever a shelter is use, the individual with the program staff either develop, or in process of utilizing a new housing plan as part of the overall service plan.
Shelter Services. C.3.17.1 The Provider shall ensure units are inspected weekly between the hours of 9:00am to 9:00pm with reasonable notice of the planned inspection to ensure minimal standards in the HSRA are met for unit upkeep.
C.3.17.2 The Provider shall ensure the STFH site meets the ICH Minimum Habitability Standards (under development), HSRA requirements, and all requirements specified in this Statement of Work.
C.3.17.3 The Provider shall ensure any inspectors performing housing or shelter inspections under this SOW are appropriately certified to conduct the specified inspection.
C.3.17.4 The Provider shall procure all applicable licenses or permits necessary to meet the code regulations required to operate the STFH site. All applicable licenses or permits shall be retained at each site.
C.3.17.5 The Provider shall maintain the STFH site in compliance with fire safety requirements, and shall maintain required documentation, including the Certificate of Occupancy, case records, and personnel records.
C.3.17.6 The Provider shall provide the CA with written procedures to ensure the health and safety of residents, staff and visitors on a daily basis within thirty (30) days of award. The procedures must address such issues as fire safety, the handling of blood borne pathogens, first aid, and cardiopulmonary resuscitation certification (CPR) to ensure the protection of staff and clients.
C.3.17.7 The Provider shall provide a written Closed Point of Dispensing (CPOD) plan to the CA within 30 days of award for the dispersal and storage of medication, as well as vaccination for clients. The CPOD shall include procedures regarding the storage and handling of medication. The Provider shall also ensure all staff members receive appropriate training on the CPOD plan, including both initial and ongoing training.
C.3.17.8 The Provider understands that authorized representatives of the District or DHS shall have the right to monitor and conduct on-site inspections at the STFH sites. The District and DHS reserve the right to conduct both announced and unannounced site visits, as deemed necessary. The Provider shall participate in and make staff and space available for all monitoring visits, and shall make available any and all requested information (including information regarding resident feedback/satisfaction) during or prior to the inspection as requested by DHS.
C.3.17.9 The Provider shall conduct quarterly fire drills across shifts in accordance to District law at the STFH sit...
Shelter Services. City shall provide the shelter services to the residents of Imperial Beach in the manner and type described below:
Shelter Services. Goal: Contractor shall increase the independence of clients by assisting them in obtaining any and all necessary support services (e.g., financial assistance, housing, medical/psychiatric services, etc.) to facilitate a higher level of self-sufficiency. Objective: A minimum of sixty-five percent (65%) of clients shall obtain stable (permanent or transitional) housing upon discharge.
