Sick Bank Guidelines. A) Upon the execution of this agreement there shall be established a Sick Leave Bank, hereafter referred to as the Sick Bank. The basic purpose of this Bank is to alleviate the effects of prolonged illness, injury, or hospitalization upon the Prairie State College Supportive Staff members. In accordance with this purpose, the Sick Bank shall not be used for single day or short term occurrences.

Sick Bank Guidelines. A sick bank shall be established for service by September 15 of each year. The sick bank guidelines are as follows:
Sick Bank Guidelines. 1. Member must be hired a minimum of two (2) years to be eligible to draw from sick bank and have donated toward the sick bank.
Sick Bank Guidelines. The Lindop School sick leave bank is open only to full-time certified personnel. To part1c1pate, employees must fill out the approved form by October 1st of each year. Employees joining for the first time shall contribute one (l) sick day to the bank from their personal accumulated sick leave. Days donated may not be withdrawn and are recorded as consumed on the individual member's official school record of accumulated sick leave. New and returning staff members may apply during the first month of their employment and shall contribute one (l) day. Prior to June 30th in their final year of service retiring teachers may donate any unused sick days to the Sick Leave Bank which have not been used for TRS service credit and for which they have not otherwise been compensated subject to the limits on the overall maximum days that may be accumulated in the Sick Leave Bank. PROVISIONS FOR USE:
Sick Bank Guidelines. Any employee may make a request(s) to the Association to withdraw days from the bank only after all regular accumulative sick/personal leave has been exhausted. Sick bank days are to only be used for illness or other medical related matters. An application to borrow sick days must accompany all requests made to borrow from the sick bank. The application should be submitted to the KHEA President prior to the absence, or within five school days of first absence. In cases of extended absences, it is the employee’s responsibility to submit medical paperwork excusing the employee from work to the business office prior to the absence, or as close to the start of the absence as possible. Failure to do so may result in a lag or loss of pay. It shall be mandatory that anyone borrowing days from the sick bank will have a gradual payback of two (2) days per year until the days are paid back. Medical appointments: Personal illness and EMERGENCY medical appointments qualify for sick leave. Teachers will attempt to schedule elective surgery during vacation periods. Medical appointments directly related to personal illness qualify for sickleave. Xxxx leave is compensable time off for illness for the teacher and his/her family. Sick leave may not be used for any other purpose than that for which it is intended. Misuse of sick leave may result in a deduction of a day’s pay for each day misused. The Association maintains the right to deny sick bank requests or ask for doctor notes in cases of reasonable suspicion of abuse. The Association maintains the right to review absence history prior to granting sick days from the bank. Upon retirement, the sick bank needs to be reimbursed in full before any sick day payout is made to the employee. KHEA Kenowa Hills Education Association APPENDIX D - 2

  • Usage Guidelines If Customer develops or has developed advertising, marketing, or promotional materials related to Products that uses Neuronetics trademarks, service marks, trade dress, other proprietary elements, or other indicia of origin controlled by Neuronetics, including logos and designs, then, before using or disseminating any such materials: (i) Customer will comply with the then-current Neuronetics branding and style guidelines and the then-current Neuronetics quality control requirements; and (ii) provide copies of such materials to Neuronetics. Neuronetics reserves the right, at any time, to: (ix) require Customer to change or discontinue Customer’s use of the Neuronetics Marks or the Marketing Material if Neuronetics, in its sole discretion, believes that such use is detrimental to Products, the Neuronetics Marks, the Marketing Material, or Neuronetics’ business interests.; and (x) inspect to ascertain that the Neuronetics Marks and the Marketing Collateral are used only for services performed using Products. Customer will promptly comply with Neuronetics’ requests regarding the use of the Neuronetics Marks and the Marketing Collateral. Customer will not adopt, use, or seek to register any trademarks, service marks, trade dress, other proprietary elements, or other indicia of origin, including logos and designs, that are confusingly similar to any of the Neuronetics Marks, and Customer will not combine any of the Neuronetics Marks with any other trademark, service mark, other mark, trade dress, other proprietary element, other indicia of origin, logo, design, word, term, device, logo or other matter. Customer will not adopt, use, or register any domain name that incorporates any of the Neuronetics Marks. Customer will not in any way use the Neuronetics Marks in a manner that could cause any confusion with, diminution in value of, or other adverse effect to the Neuronetics Marks. All goodwill derived from the use of the Neuronetics Marks will inure solely to the benefit of Neuronetics.

  • Guidelines The Office of State Procurement adheres to all guidelines set forth by the State and Federal Government concerning The Americans with Disabilities Act (ADA) as well as all mandated fire codes.
