Supplemental Disability. During the Employment Term, the Company shall provide Executive with supplemental disability coverage to ensure that total disability benefits are equivalent to sixty percent (60%) of the Base Salary, up to a maximum benefit of $35,000 per month, pursuant to the policy in effect as of immediately prior to the Effective Date or, as determined by the Company, a replacement policy that provides equivalent benefits.
Supplemental Disability. The Company agrees to provide to the ----------------------- Employee disability insurance coverage in an amount equal to 60% of Employee's base salary.

Supplemental Disability. Section 1 In the event that a more permanent employee who has been employed ninety (90) days or more by the Employer becomes disabled, the following shall apply:
Supplemental Disability. The City will contribute one hundred percent (100%) of the premium of a supplemental disability insurance program for each member. These contributions will be paid toward the Guild-sponsored disability insurance.
Supplemental Disability. Employer will reimburse Executive for existing annual supplemental disability premiums which currently approximate $10,000.00.
Supplemental Disability. The Executive shall be eligible for Company-paid long-term disability coverage of up to 50% of his Base Salary and additional coverage up to 65% of his Base Salary at his own expense.
Supplemental Disability. QTS LLC will reimburse Executive for existing annual supplemental disability premiums which currently approximate $10,000.00.
Supplemental Disability. Executive shall be reimbursed by the Bank for the premium cost of supplemental disability insurance purchased by Executive, such reimbursement not to exceed $2,800 per calendar year.
Supplemental Disability. The Authority shall provide to employees a Supplemental Disability Plan (Plan II, Option II) which provides for a $750.00 per month benefit, the cost of the premium to be paid by the Authority.
