THE HOSPITAL AGREES. 1. To provide Resident with the benefits listed on the Benefits Summary Sheet listed on Attachment A, attached hereto and made a part hereof. SRHS reserves the right to modify such benefits if SRHS deems such modifications necessary. 2. To provide written rules and regulations clearly defining the duties and privileges to which Resident is expected to abide. Included in the rules and regulations are descriptions of the educational programs; information about reappointment, grievances, and disciplinary hearings; the Hospital’s sexual harassment policy; the Hospital’s impairment policy, accommodation for disabilities, residency closure/reduction policy, and information on the professional liability coverage. 3. To provide the Resident access to appropriate and confidential counseling, medical and psychological support services. 4. To provide professional liability insurance coverage for Resident while rendering services on behalf of SRHS, pursuant to the applicable provisions of this Agreement. SRHS self-insures or secures coverage through a commercial liability carrier, of its choice, in amounts determined by SRHS to be adequate to cover such liability. The professional liability coverage provided by SRHS is effective for physicians employed by SRHS and the Residents. The professional liability coverage does not require “tail coverage or “prior acts” coverage. 5. To provide the Resident with performance evaluations in the Resident’s training program, including discussion with Program Director, or the Director’s designee, on the Resident’s overall progress toward his/her educational objectives, on a regular basis. 6. To provide access to eligibility for certification by the relevant certifying board, and to present a proper certification upon completion of satisfactory period of residency.
THE HOSPITAL AGREES. (1) to designate coordinators who will be responsible for coordinating placements with the University coordinators; (2) that in cooperation with the University coordinator, to determine the number of students for which clinical or other experience and necessary facilities can be provided, the desirable staff-patient ratio, and the number of students that can be assigned to specific clinical areas; (3) to advise the University coordinator before the commencement of a placement if educational objectives cannot be met by the Hospital; (4) to advise the University as early as possible in advance of a scheduled placement, of any placement cancellations or changes; (5) to participate in the selection of clinical assignments as appropriate; (6) to the extent and in the manner agreed to by the University and Hospital coordinators prior to the commencement of a placement, to provide practice and observational experiences, and instruction, training and guidance for the students by Hospital staff qualified in the discipline in which the student is training, to meet the objectives of a particular placement and to provide such performance evaluations and assessments as required for a particular program; (7) to ensure that there is on-site supervision of students' clinical activities by qualified Hospital staff if University faculty are not on-site with the students during the placement; (8) that in cases where University faculty are on-site with the students during all or part of the placement term, the University and Hospital have a shared responsibility for taking necessary measures to ensure that there is appropriate supervision of the students' activities by qualified clinical personnel throughout the placement term; (9) to provide opportunities for clinical instruction that are sufficient in extent and variety to meet the objectives of a particular placement in those situations where University instructors are instructing the students at the Hospital.

  • Hospital This plan covers behavioral health services if you are inpatient at a general or specialty hospital. See Inpatient Services in Section 3 for additional information. This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.

  • Hospital Services The Hospital will: 6.1.1 achieve the Performance Standards described in the Schedules and the HSAA Indicator Technical Specifications; 6.1.2 not reduce, stop, start, expand, cease to provide or transfer the provision of Hospital Services to another hospital or to another site of the Hospital if such action would result in the Hospital being unable to achieve the Performance Standards described in the Schedules and the HSAA Indicator Technical Specifications; and 6.1.3 not restrict or refuse the provision of Hospital Services that are funded by the Funder to an individual, directly or indirectly, based on the geographic area in which the person resides in Ontario, and will establish a policy prohibiting any health care professional providing services at the Hospital, including physicians, from doing the same.

  • EMPLOYEE DEVELOPMENT AND TRAINING 1. The State agrees to provide advice and counseling to employees with respect to career advancement opportunities and agency developments which have an impact on their careers. 2. Regular review of its job-related and career development and training programs will be made by the State in order to provide suitable programs for employees covered by this Agreement. When undertaking any such review, the State shall notify employees of such review and take into account suggestions and proposals made by employees. 3. Employees shall be given a reasonable notice of applicable, development and training programs available. Such notice shall include an explanation of the procedure for applying for the program. Notices of development and training programs shall be posted for reasonable periods in advance on bulletin boards at applicable work locations within the agencies involved. An appointing authority shall make every effort to permit employees' participation in such career development and training programs. Participation in any training inside or outside of work hours which is required by the State as a condition of fulfilling the requirements of the employee's job, or any in-service State training which is conducted or undertaken during normally scheduled work hours will be considered as time worked. 4. The State shall pay tuition, course-related fees, other approved course required costs and for necessary travel and lodging pursuant to established policies and procedures.

  • Formal Agreement If the above Agreement and relating exhibits thereto are acceptable, would you please execute the acceptance and acknowledgment hereinafter provided, upon which this letter will become a binding agreement between us.

  • Diagnostic Services Procedures ordered by a recognized Provider because of specific symptoms to diagnose a specific condition or disease. Some examples include, but are not limited to:

  • Infertility Services Freezing, storage and thawing of embryos, sperm, or other tissues, for future use, unless the freezing, storage and thawing is needed due to potential iatrogenic infertility as described in Infertility Services in Section 3. • Reversal of voluntary sterilization or infertility treatment for a person that previously had a voluntary sterilization procedure. • Fees associated with finding an egg or sperm donor, related storage, donor stipend, or shipping charges. • Services related to surrogate parenting, when the surrogate is not a member of this

  • Total Agreement 35.01 This Agreement represents the entire agreement between the Employer and the Union and unless specifically and expressly set forth in the express written provisions for this Agreement, all rules, regulations, benefits and practices previously and presently in effect may be modified or discontinued upon advance notification to the Union.

  • NOTICE OF FINAL AGREEMENT THIS AGREEMENT AND THE OTHER LOAN DOCUMENTS REPRESENT THE FINAL AGREEMENT BETWEEN THE PARTIES, AND THE SAME MAY NOT BE CONTRADICTED BY EVIDENCE OF PRIOR, CONTEMPORANEOUS OR SUBSEQUENT ORAL AGREEMENTS BETWEEN THE PARTIES. THERE ARE NO UNWRITTEN ORAL AGREEMENTS BETWEEN THE PARTIES.

  • Your Agreement If one or more Potential Changes in Control occur during the Term of this Agreement, you agree not to resign for at least six full calendar months after a Potential Change in Control occurs, except as follows: (a) you may resign after a Change in Control occurs; (b) you may resign if you are given Good Reason to do so; and (c) you may terminate employment on account of retirement on or after 65 or because you become unable to work due to serious illness or injury.

  • Rental Agreement All of the Roommates agree to be bound by all of the terms of the Rental Agreement.
