Basic Care. The following are the expenses eligible for reimbursement and subject to the deductible coinsurance and maximums outlined in the Benefit Schedule, if applicable.

Basic Care. In the event that the User, who is a natural person, does not meet the relevant conditions for the provision of simplified care according to Article 3.1. of this Agreement, the User in question will be provided with basic care. For the purposes of providing basic care, the User, who is a natural person, is obliged to provide FUMBI with additional information or documents in the following scope in addition to the scope of the Identification Data before the further provision of the Services: • a copy of the identity document; • confirmation of permanent residence or other authorized residence (duly issued invoice for the supply of electricity, or a bank statement with identification data and the User's matching address, extract from the population register); • a photograph of the User (selfie) together with clearly legible data from the identity document and a recognizable face of the User; and • information on the origin of funds. FUMBI is obliged to provide basic care within the meaning of this Agreement in accordance with the terms of the AML Act and the AML Directive to a User who is a legal entity. For the purposes of providing basic care to the User, who is a legal entity, FUMBI is obliged to request the following information or documents: • business name; • company ID number; • registered office address; • identification data of the entry in the relevant register; • a copy of the identity documents of the statutory bodies and ultimate beneficial owners of the User's and their data within the scope of their Identification Data pursuant to Article 3.1. of this Agreement; • confirmation of the registered office address (duly issued invoice for the supply of electricity, or a bank account statement stating the identification data and the User's matching address, an extract from the Commercial Register or a similar register); • a photograph of the person (selfie) carrying out the opening of the User's Account together with clearly legible data from the identity document and a recognizable face, as well as the Identification Data of the person; • information on the ownership structure of the User in the scope of the Identification Data according to Article 3.1. of this Agreement, if the shareholders are natural persons and if the shareholders of the User are legal entities, then to the extent of: i) business name, ii) company ID number, iii) address of the User's shareholder’s registered office, iv) identification data of the User's shareholder concerning e...

Incident/Accident Reports 1304.22 As needed The Teacher or Assistant Teacher will provide documentation for the parent of any child who was involved in an incident resulting in injury or exposure. Head Start will also receive notification of incidents or accidents for tracking and documentation purposes. This tracking information will be shared with the building administrator and the teaching staff on a quarterly basis

All tuition and fees are non-refundable and non-transferable. Because we operate on a cash basis, it is essential that all parents pay tuition in a timely manner. We may refuse to admit your child to school if you fall behind in your payment. Financial emergencies must be discussed with the Director, so that a method of payment may be arranged. Enrollment may be terminated if full payment is not received by the sixteenth of the month and prior arrangements have not been made. A $25.00 service charge will be billed for each returned check. We will re-evaluate our budget annually and as the need arises. You will be given a written thirty-day notice of any change in our tuition. Families should expect an increase of 5%-10% per year to keep pace with increasing costs. By signing this agreement, the Parent/Guardian agrees to pay in full all amounts due, regardless of the child’s custody arrangement.

  • Urgent Care This plan covers services received at an urgent care center. For other services, such as surgery or diagnostic tests, the amount that you pay is based on the type of service being provided. See Summary of Medical Benefits for details. Follow-up care (such as suture removal or wound care) should be obtained from your primary care provider or specialist.

  • General Care ● Do not do anything to the District-issued Device that may permanently alter them in any way. ● Do not remove any serial numbers or identification marks placed on the District-issued Device. ● Keep the District-issued Device clean (e.g., do not eat or drink while using the Computing Device). ● TJUHSD does not provide backup services for any District-issued Device. Students should not store important documents on District-issued Device. District technicians will not attempt to recover lost data on District-issued Device. Carrying the District-issued Device: ● It is recommended to use a padded backpack or a case to transport the District-issued Device.

  • Emergency Care If you need emergency care, call 911 or go to the nearest hospital emergency room. If you are traveling outside our service area and need urgent care, call the Customer Service number provided in the chart above or visit our website and use the “Find A Doctor” feature to find a BlueCard provider.

  • Vision Care Effective July 1, 2000, the District shall provide all full-time employees and their dependents with Vision Service Plan (VSP) Plan C. This plan shall provide for a comprehensive exam and new lenses every 12 months, and new frames every 12 months. All other services will be pursuant to the standard VSP plan description, except that it will reimburse up to $50 for examinations by non-panel providers. There shall be a $10 annual deductible on materials only. In addition, the following vision plan enhancements shall take place effective July 1, 2000: $60 wholesale frame allowance; computer glasses; progressive lenses, tints, and UV coatings.

  • Child Care A. Employees employed as of March 1 who meet the following criteria shall be eligible for a lump sum payment each year. Eligible employees may apply for this payment between March 1 and April 15 of each year. Payment shall be made within thirty (30) days of receipt of the completed application. Any application received after April 15 will be considered on a case by case basis and shall not be arbitrarily rejected.

  • Medical Care The Parents must comply with the School Welfare Officer's recommendations which may include a reasonable decision to release the Pupil home or to his / her education guardian when s/he is unwell.

  • Special Services Should the Trust have occasion to request the Adviser to perform services not herein contemplated or to request the Adviser to arrange for the services of others, the Adviser will act for the Trust on behalf of the Fund upon request to the best of its ability, with compensation for the Adviser's services to be agreed upon with respect to each such occasion as it arises.

  • Patient Care Resident shall participate in safe, effective, and compassionate patient care, under supervision, commensurate with Resident's level of advancement and responsibility.
