Birthday Parties. The Hirer will complete a Booking Facility Application Form and children/staff ratios will be agreed in advance. Full payment is required within 7 days of the booking being confirmed. One week’s notice is required for a Birthday Party to the cancelled. Re booking and postponing Birthday Parties is at the discretion of Holy Trinity School/School/ Staff/Caretaker. Cheques should be made payable to ‘Holy Trinity Catholic School’ The Hirer is responsible for providing any information relating to external Birthday entertainers. Companies will be required to provide public liability and relevant insurance documents, requested by Holy Trinity School/School/Staff/Caretaker. – See section 10 (page 10)

Birthday Parties. Pacific SLSC does not hire the club for 18th Birthday parties. Pacific does hire the club for 21st Birthday Parties only with the engagement of Professional Security Staff who must be present for the entire function. Such costs for Professional Security Staff will be billed to the hirer.

Rental fees include use of the lobby area for one-hour prior to open swim, two complimentary adult swim admissions, one birthday boy/girl admission, balloon for the birthday person, tables and seating for up to 16 people. Birthday Parties. Children may celebrate their birthdays by bringing refreshments to school. Refreshments should be limited to simple snacks. For Pre-kindergarten grades, please no cupcakes. As we have found they are too messy. Arrangements need to be made in advance with the classroom teacher. Treats will be shared during the last half hour of the school day. Favors are not permitted. Invitations for any party to be held out-of-school may not be distributed at school unless the entire class is included, or all of a particular gender (for slumber parties, etc.). Early dismissal for parties is not granted. Balloons, flower or cookie arrangements delivered to the school will be held in the school office for pick up at the end of day.

Parents must get approval from the teacher at least a week in advance, securing a day. Parents may not bring in home cooked foods, but must purchase foods from a store with ingredients clearly marked on the packaging. Parents must also provide plates, napkins, utensils, and if desired, individual drinks (water or juice box only) for each student. CHILD ABUSE: State law requires that teachers, administrators and other school personnel must report suspected cases of abuse, abandonment, or neglect to the County Department of Social Services.

  • CONTACTS BETWEEN THE PARTIES 15.1 Each Party shall update its own contact information and escalation list and shall provide such information to the other Party for purposes of inquiries regarding the implementation of this Agreement. Each Party shall accept all inquiries from the other Party and provide a timely response. CenturyLink will provide and maintain its contact and escalation list on the CenturyLink Website, and any updates also will be provided on the Website.   • Submitting Loop Makeup Service Inquiries 2.9.2.1 C.M. may obtain LMU information by submitting a mechanized LMU query or a Manual LMUSI. Mechanized LMUs should be submitted through BellSouth's OSS interfaces. After obtaining the Loop information from the mechanized LMU process, if C.M. needs further Loop information in order to determine Loop service capability, C.M. may initiate a separate Manual Service Inquiry for a separate nonrecurring charge as set forth in Exhibit A of this Attachment.

  • Birth Mother A pregnant employee shall be entitled to up to seventeen (17) consecutive weeks of maternity leave and up to thirty-five (35) consecutive weeks of parental leave, all without pay.
