Categories of Service. 1. The following areas of public health service shall annually constitute the Service (this shall not include any elements of the activities listed below that are currently undertaken by either Council‟s employees who are not engaged within the Public Health Service). 2. Each Council shall annually determine and specify the Public Health Services to be provided in their respective areas (the Commissioning Requirements) for the stated year taking account of the Public Health Outcomes Framework, guidance issued by the Department of Health, the Joint Strategic Needs Assessment for the area and population and the Joint Health and Wellbeing Strategy 3. The Commissioning Requirements for the shared public health service will be split into Barnet and Harrow to ensure transparency for each borough‟s public health budget. 4. The Commissioning Requirements shall specify and distinguish between the following categories : a. mandatory services b. additional services c. excluded services 5. The annual Commissioning Requirements for each borough shall also specify the performance outcomes and targets to be achieved. 6. Staff costs for the shared public health service will be split between the Parties based on the annually agreed schedule of staffing costs submitted as part of the annual Commissioning Requirements in accordance with Appendix 2 of this Agreement. 7. The annual Commissioning Requirements shall include a projection of the related staffing costs, contract costs and overhead costs. 8. The draft Barnet Commissioning Requirements must be submitted to the Harrow at a time consistent with the overall budget and Council Tax setting arrangements of each Party. 9. Based on the draft Commissioning Requirements of both Councils the host borough will calculate the cost of hosting and delivering the service annually. 10. The annual total cost of delivering the public health service Commissioning Requirements for each Borough must cost no more than the allocated funding made available by the respective Borough. 11. During any financial year either Council may request the provision of additional Commissioning Requirements provided that adequate financial resources are identified within current budgetary provision or additional identified financial resources are made available.

Categories of Service. The following areas of the HB SNT Service shall constitute the Service, and will be provided by Harrow (in its role as partnership shared services provider). It is accepted by both Parties that during the Agreement period there may be agreed variations to services in accordance with Clause 20. • Formal advice within the area of professional expertise of HB SNT Services to Xxxxx Council Officers, Council, Cabinet, Committees and other decision making bodies of Harrow or Xxxxx; • Formal advice to Xxxxx officers through agreed and secure protocols; Harrow shall provide the Service in accordance with all relevant primary and secondary Legislation including but not limited to the Education Act 1996, Education and Inspections Act 2006 and the Equality Act 2010.

  • UNITS OF SERVICE CONTRACTOR shall provide, pursuant to the Contract, no more than 24 Recovery Residence Units of Service, per Resident within a twelve (12) month period, unless otherwise 25 authorized in writing by ADMINISTRATOR. ADMINISTRATOR may increase or decrease the 26 maximum length of stay in writing for specific populations as needed to accommodate the demand for 27 Recovery Residence services. Residents may be allowed to stay up to an additional thirty (30) calendar 28 days beyond the maximum allowable length of stay with prior approval from ADMINISTRATOR.

  • Level of Service Complex case management with a provider focus is appropriate for members who either choose not to be actively involved or are unable to actively participate in their health care. Complex case management targets members with two (2) or more disease states who need assistance with care coordination, making preventive care appointments, or accessing care to address the members’ chronic health conditions or members who have had an inpatient hospital stay in the last ninety (90) days or members with high dollar claims of over fifty thousand dollars (>$50,000) in six (6) months. The focus is on working with the providers to meet the needs of the individual through communication with the PMP (if applicable), other providers, and the member’s natural support system. The goal is to help members gain optimum health or improved functional capability, in the right setting and in a cost-effective manner. Complex case management with provider focus is the active coordination by the Contractor of care and services between providers while navigating the extensive systems and resources required for the member. It involves comprehensive assessment, determination of available benefits, development and implementation of a complex case management plan directed at the chronic health conditions. At a minimum, the Contractor must provide complex case management services for members discharged from an inpatient psychiatric, drug overdose, or substance abuse hospitalization, for no fewer than ninety (90) calendar days following that inpatient hospitalization discharge. The Contractor must also provide complex case management services for any member at risk for inpatient psychiatric or substance abuse re -hospitalization. Care managers must contact members during an inpatient hospitalization or as soon as practicable upon receiving notification of a member’s inpatient behavioral health hospitalization. The care manager must work with the hospital discharge planner, provider case manager and/or natural supports (i.e. family) to ensure that an outpatient follow-up appointment is scheduled to occur no later than seven (7) calendar days following the inpatient behavioral health hospitalization discharge and transportation is not a barrier to attending the appointment. Complex case management with provider focus includes all of the services and benefits from disease management and care management. In addition,
