Children’s System of Care Governance Structure. Contractor shall establish and maintain a governance structure to support the development of a functional System of Care in its service area in accordance with the Portland State University System of Care established best practices. The governance structure shall incorporate the following functions as described in the Best Practice Guide located at xxxxx://xxx.xxx.xxx/ccf/best-practice-guide Executive Council, Advisory Committee, Practice Level Workgroup, and Wraparound Review Committee. As long as the functions are carried out, up to 2 committees and councils may be combined. a. The functions of SOC Executive Council are to develop and approve related policies, shared decision-making regarding funding and resource development, review of project outcomes, and identify identification of unmet needs in the community to support the expansion of the service array. b. The SOC Advisory Committee (previously designated as the Steering Committee) advises policy development, financing, implementation, reviews fidelity and outcomes, and provides oversight using a shared decision-making model. It shall respond to the barriers which the Practice Level Workgroup cannot resolve, making recommendations to the Executive level as needed. The SOC Advisory Committee consists of representatives of the Contractor, its Subcontractors, partner agencies, local service providers funded by DHS, criminal justice and other agencies including, young adults or Young Adult Peer Support, families and Family Partners, and advocacy organizations and relevant cultural representation. c. The functions of the Practice Level workgroup are to review Wraparound practice barriers, remove barriers when possible, and submit barriers that remain unresolved to the SOC Advisory Committee and/or Executive Council for resolution and/or advancement to the State System of Care Steering Committee. d. Contractor shall convene a Wraparound Review Committee for the purpose of reviewing all referrals submitted to the Wraparound program and determine eligibility for entry into the program. This committee’s functions are defined in Exhibit A 3 (tttt). 1. Covered Services a. For purposes of routing Verizon traffic to ECI, the subtending arrangements between ECI Tandem Switches and ECI End Office Switches shall be the same as the Tandem/End Office subtending arrangements that ECI maintains for the routing of its own or other carriers’ traffic.

  • Quality Management System Supplier hereby undertakes, warrants and confirms, and will ensue same for its subcontractors, to remain certified in accordance with ISO 9001 standard or equivalent. At any time during the term of this Agreement, the Supplier shall, if so instructed by ISR, provide evidence of such certifications.
