Wraparound Services. High Fidelity Wraparound Services - The Wraparound Services model delivers services to children and families who are Medi-Cal beneficiaries with severe and multiple problems often being served by multiple agencies. Wraparound services includes treatment services, personal support services, and any other support necessary to maintain the child/youth in the family home or at the lowest level of appropriate care. Services are delivered/developed through an interagency collaborative approach that includes family participation and the family as an active team member.
Wraparound Services. The Employer shall maintain minimum staffing ratios in compliance with the recommendations of the professional organizations as listed below.
Wraparound Services. Wraparound Services must be prior authorized see Network Provider Manual, which is located on COUNTY's website, xxxxxxxxxxxx.xxx, under "Behavioral Health Services." CONTRACTOR shall take all appropriate actions to ensure that no person employed by CONTRACTOR to provide services under this Agreement shall have committed, or been convicted of, any form of child abuse.
Wraparound Services. (1) Contractor shall administer fidelity Wraparound services to not less than 205 Members per month as described in the OHA System of Care Wraparound Initiative (SOCWI) guidance document found at: xxxx://xxx.xxxxxx.xxx/oha/hsd/amh/pages/index.aspx.Contractor shall ensure Fidelity Based Wraparound Care Coordination for Members 17 and younger for any of the following situations:
(a) Placement in Secure Adolescent Inpatient Program (SAIP), Secure Children’s Inpatient Program (SCIP),
(b) Psychiatric Residential Treatment Services (PRTS) or the Commercial Sexually Exploited Children’s residential program funded by OHA.
(c) Children meeting local/regional System of Care Wraparound Initiative entry criteria.
(d) Contractor shall ensure subcontractors (Day Treatment, PRTS, SAIP and SCIP providers) have an understanding of Wraparound values and principles and the provider’s role within the child and family team. Contractor shall ensure subcontractors collaborate and participate in the Wraparound process.
(2) Contractor shall ensure that the ratio of care coordinators, family support specialists, and youth support specialists to families served is no greater than 1:15.
(3) Contractor shall ensure that core values and principles described in ORS 418.977 are followed in delivering SOC Wraparound services and supports, Wraparound Care Coordination and Team Facilitation.
(4) Contractor shall ensure that SOC Wraparound services and supports include Family Support Specialist, (Family Partners) and Young Adult Support Specialists (Youth Partners), as appropriate. The Family Support Specialist and Young Adult Support Specialists must have experience navigating the mental/behavioral health, child welfare, or juvenile justice system with a child or youth, and be active participants in the
(5) Contractor shall maintain a Wraparound Review Committee which shall review all referrals submitted to the Wraparound program and determine eligibility for entry into the program.
(6) Contractor shall submit any consistently identified Wraparound or other system barriers to the Practice Level Workgroup up through the Executive Council if needed and submit barriers that remain unresolved to the State System of Care Steering Committee.

Wraparound Services. After School Programs
Wraparound Services. Beginning in the 2019-2020 school year, each School will maintain appropriate staffing ratios for Counselors and licensed School Social Workers. For Nurses and Psychologists, the Schools will offer services consistent with the staffing ratios recommended by the professional organizations listed below:

Wraparound services include treatment services, personal support services, and any other support necessary to maintain the child/youth in the family home or at the lowest level of appropriate care. Services are delivered/developed through an interagency collaborative approach that includes family participation and the family as an active team member. Wraparound aims to provide Medi- Cal/CalAIM compliant mental health services to program-enrolled youth whenever possible (including individual, family, and/or group therapy, collateral support, targeted case management, individual rehabilitation, ICC, IHBS, and medication management). MHSA funding would be utilized to provide other targeted interventions related to the youth’s wellbeing and successful completion of their Wraparound service plan not captured under the SMHS/EPSDT service definitions. Non Mental Health Supportive Services and Goods funding can also be access for supports, and services to further the success of the Client Services Plan through the approval of a CFTM. Wraparound will serve approximately 60 youth annually, with 30 served at any given time.

Wraparound services are developed through a team-planning process, where a team of individuals who are relevant to the well-being of the youth (such as family members, service providers, teachers, and representatives from any involved agency) collaboratively develop and implement an individualized plan of care, known as a wraparound plan.
