CIVIL SERVICE - LONG TERM DISABILITY INCOME PLAN. It is understood that the elimination period for the Long Term Disability Income Plan is the greater of one hundred and twenty (120) calendar days or the exhausting of the employee’s Income Protection Bank to a maximum of two hundred and eight (208) days. An employee may use up to five (5) days of Income Protection in any one (1) calendar year to provide care in the event of an illness of a spouse, child, or parent of the employee. An employee who is to work by reason of accident or illness which is not covered by income protection shall, upon providing an acceptable medical certificate attesting to her inability to perform the normal duties of her job, be granted an unpaid leave of absence for a period of one (1) month per year of service up to a maximum of nine (9) months. An employee who is able to resume work following a period of absence which exceeds one (1) shall notify the Employer of her ability to resume work at least five (5) calendar days prior to the date of her intended return. For claims occurring on or after the date of ratification (May 6,2004) an employee who is unable to work by reason of accident or illness which is not covered by income protection shall, upon providing an acceptable medical certificate attesting to her inability to perform the normal duties of her job, be granted an unpaid leave of absence for a period of one (1) month per year of service up to a maximum of twelve (12) months. An employee who is able to resume work following a period of absence which exceeds one (1) month shall notify the Employer of her ability to resume work at least five (5) calendar days prior to the date of her intended return. If the employee is unable to resume her normal duties at the expiry of her leave of absence, her employment may, at the discretion of the Employer, be considered terminated. An employee so terminated who applies for re-employment with the Employer immediately upon recovery from her illness, shall be given preference over new applications in hiring, subject to her providing an acceptable medical certificate. Where an employee is injured on thejob and is required to leave for medical treatment is sent home by management due to the injury, the employee shall incur no loss in regular pay and benefits for the day on which the accident occurs.

The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Long Term Disability Benefit In the event an employee, while covered under this plan, becomes totally disabled as a result of an accident or a sickness, then, after the employee has been totally disabled for seven (7) months, including periods approved in Section 1.3(a) and (c), he/she shall be eligible to receive a monthly benefit as follows:
