Colleague Feedback Sample Clauses

The Colleague Feedback clause establishes a process for obtaining and incorporating input from colleagues or team members regarding a particular matter, such as performance, project progress, or decision-making. Typically, this clause outlines how feedback should be solicited, the format in which it should be provided, and any timelines or confidentiality requirements associated with the process. Its core practical function is to ensure that diverse perspectives are considered, promoting transparency and informed decision-making within an organization or team.
Colleague Feedback. 17.13.1 The Colleague feedback forms will be used to gather input from colleagues. 17.13.2 The supervisor shall distribute the colleague feedback forms to the persons identified during the initial meeting and collect the completed forms. 17.13.3 Colleague feedback forms shall include the name and signature of the person completing the form. Anonymous forms shall be discarded and not considered for evaluation. 17.13.4 The bargaining unit member undergoing evaluation shall not have a right to see the names of the person(s) who completed the feedback form except if disclosure is required during a formal grievance proceeding. The supervisor shall ensure that the bargaining unit member undergoing evaluation does not view the name(s) of the person(s) who completed the feedback forms during the regular evaluation process. 17.13.5 Any reference to colleague feedback in the final written evaluation shall be based solely upon information gathered from the colleague feedback forms and/or (a) substantiated complaint(s).
Colleague Feedback. The Organization values the benefit that employees can gain from feedback from colleagues other than their supervisor. A standardized colleague evaluation form shall be used for employees to receive feedback from colleagues with whom they work closely on tasks and/or projects related to their job duties. The colleague feedback process will not replace the annual performance review between an employee and their supervisor, but is a tool that complements the process by collecting and providing greater feedback for employees upon their request. Employees may request voluntary feedback from up to a maximum of two (2) colleagues. Requests for colleague feedback will not be unreasonably denied, and no employee shall be asked to provide feedback to more than three (3) colleagues in a year. Colleague feedback should be provided during employees’ regular work schedules, and employees may have up to three (3) weeks to complete the colleague feedback. While all employees are encouraged to participate in the colleague feedback process, employees can elect to opt out of the process at any stage.
Colleague Feedback. (a) Colleague feedback is mandatory as evidence of teamwork in all cases and of leadership effectiveness where the leadership is being exercised in informal contexts. The standard feedback form also covers these dimensions. (b) Colleagues will generally be people with whom the applicant works on a regular basis (or has worked on a project or Committee, either internal to Otago Polytechnic or external) and who have been able to observe how the applicant goes about his/her work. (c) Colleague feedback gathered by way of standard survey must be collected through an independent party, not directly by the staff member. Respondents must be assured of their anonymity. (d) Where “Leadership and Service” is the chosen career path and the staff member is in a formal leadership role, feedback is expected from all colleagues for whom the staff member has leadership responsibilities.
Colleague Feedback. 17.13.1 Colleague feedback forms in the appendix are used to gather input from colleagues and staff. 17.13.4 The bargaining unit member undergoing evaluation shall not have a right to see the names of the person(s) completing the feedback. The supervisor shall ensure that the bargaining unit member undergoing evaluation does not view the name(s) of the person(s) completing the feedback forms.
