Contract Monitoring and Quality Assurance. The Contractor must: (a) Use contract management oversight procedures and carry out activities to ensure contract compliance of EIPAs under contract with the CLA. (b) In collaboration with the DCYF, assist with data collection and conduct compliance and quality reviews with EIPAs under contract with the CLA to ensure compliance with Part C of IDEA and Washington State’s Annual Performance Report (APR) compliance and performance indicators, provision of quality services, use of scientific, research, evidence-based and/or promising practices as appropriate and to support overall program improvement. (c) In collaboration with the DCYF provide training and TA to support EIPAs in developing and implementing the DCYF approved improvement plans and corrective action plans related to the DCYF monitoring that address root causes impacting compliance and/or performance. (d) In collaboration with the DCYF, participate in monitoring and quality assurance activities conducted by the DCYF for EIPAs under contract with the CLA. Monitoring activities may include but are not limited to fiscal, dispute resolution, data management, procedural safeguards and systems for training and technical assistance. (e) Use data to assist EIPAs progress toward improvement and/or compliance.

Contract Monitoring and Quality Assurance. The Contractor must: (1) Use contract management oversight procedures and carry out activities to ensure contract compliance of ESIT Provider Agencies under contract with the CLA. (2) Assist DCYF with data collection and conduct compliance reviews with ESIT Provider Agencies under contract with the CLA to ensure performance indicator targets are being addressed including verification of 100% compliance with compliance indicators and verification of progress moving towards outcome indicator targets that are part of the Washington State’s State Performance Plan. (3) Consistent with WAC 110-400, State Policies and Procedures, and written Guidance Memos issued by the ESIT State Leadership Office and published on the DCYF website, conduct compliance reviews with ESIT Provider Agencies under contract with the CLA to ensure compliance with Part C of IDEA, provision of quality Early Support Services, use of scientific, research, evidence-based and/or promising practices as appropriate and to support overall program improvement. Activities must include the following components: (a) Single Monthly Count Verification (b) Payor of Last Resort verification (c) System of Payments and Fees Verification (d) Contract Deliverables Verification (e) Services Delivered Verification (f) Correction of non-compliance (4) Provide the DCYF access, upon request, to all records associated with compliance and performance review of ESIT Provider Agencies and ESIT Service Providers under contract with the CLA. (5) Provide training and TA to support ESIT Provider Agencies in implementing the DCYF Determination Levels Actions and Supports Rubric, approved improvement plans and corrective action, correcting non-compliance found in contract compliance monitoring, and/or to address known performance concerns. (6) In collaboration with the DCYF, participate in the development of the new ESIT Statewide Integrated Monitoring System (E-XXXX) in support of identifying opportunities for alignment and integration of Systems Analysis Program Reviews with other state agency monitoring requirements.   • COUNTY’S QUALITY ASSURANCE PLAN The County or its agent will evaluate the Contractor’s performance under this Contract on not less than an annual basis. Such evaluation will include assessing the Contractor’s compliance with all Contract terms and conditions and performance standards. Contractor deficiencies which the County determines are severe or continuing and that may place performance of the Contract in jeopardy if not corrected will be reported to the Board of Supervisors. The report will include improvement/corrective action measures taken by the County and the Contractor. If improvement does not occur consistent with the corrective action measures, the County may terminate this Contract or impose other penalties as specified in this Contract.

  • Program Monitoring and Evaluation The Recipient shall prepare, or cause to be prepared, and furnish to the Association not later than six months after the Closing Date, a report of such scope and in such detail as the Association shall reasonably request, on the execution of the Program, the performance by the Recipient and the Association of their respective obligations under the Legal Agreements and the accomplishment of the purposes of the Financing.”

  • EMHS Governance, Safety and Quality Requirements 4.1 Participates in the maintenance of a safe work environment. 4.2 Actively participates in the Peak Performance program. 4.3 Supports the delivery of safe patient care and the consumers’ experience including participation in continuous quality improvement activities in accordance with the requirements of the National Safety and Quality Health Service Standards and other recognised health standards. 4.4 Completes mandatory training (including safety and quality training) as relevant to role. 4.5 Performs duties in accordance with the EMHS Vision and Values, WA Health Code of Conduct, Occupational Safety and Health legislation, the Disability Services Act and the Equal Opportunity Act and Government, WA Health, EMHS and Departmental / Program specific policies and procedures.

  • Monitoring and Review The Provider agrees to allow access to the Department for purposes of monitoring and review. This access includes but is not limited to client records, fiscal records, staffing records, policy and procedural manuals, facilities, staff, and children in care of the Department. The Department will conduct quality reviews, which may include site-based quality review visits.

  • Quality Assurance/Quality Control Contractor shall establish and maintain a quality assurance/quality control program which shall include procedures for continuous control of all construction and comprehensive inspection and testing of all items of Work, including any Work performed by Subcontractors, so as to ensure complete conformance to the Contract with respect to materials, workmanship, construction, finish, functional performance, and identification. The program established by Contractor shall comply with any quality assurance/quality control requirements incorporated in the Contract.

  • Quality Assurance The parties endorse the underlying principles of the Company’s Quality Management System, which seeks to ensure that its services are provided in a manner which best conforms to the requirements of the contract with its customer. This requires the Company to establish and maintain, implement, train and continuously improve its procedures and processes, and the employees to follow the procedures, document their compliance and participate in the improvement process. In particular, this will require employees to regularly and reliably fill out documentation and checklists to signify that work has been carried out in accordance with the customer’s specific requirements. Where necessary, training will be provided in these activities.

  • Monitoring and Evaluation a. The AGENCY shall expeditiously provide to the COUNTY upon request, all data needed for the purpose of monitoring, evaluating and/or auditing the program(s). This data shall include, but not be limited to, clients served, services provided, outcomes achieved, information on materials and services delivered, and any other data required, in the sole discretion of the COUNTY, that may be required to adequately monitor and evaluate the services provided under this Contract. Monitoring shall be performed in accordance with COUNTY’S established Noncompliance Standards, a copy of which is attached hereto and incorporated by reference as Attachment “C”. b. The AGENCY agrees to permit persons duly authorized by the COUNTY to interview any clients and all current and/or former employees of the AGENCY to be assured of the AGENCY’S satisfactory performance of the terms of this Contract. c. Following such evaluation, monitoring, and/or audit, the COUNTY will deliver a report of its findings and recommendations with regard to the AGENCY’S conformance with this Contract’s terms and conditions to the AGENCY and/or Board of Directors’ President, and members, whenever applicable. If deficiencies are noted, a written notice of corrective action will be issued to the AGENCY which will specify deficiencies and provide a timeline for correction of those deficiencies. Within the designated timeframe in the written notice of corrective action, the AGENCY shall submit to the COUNTY’S CCC manager (“Manager”), or their designee, a corrective action plan to rectify all deficiencies identified by the COUNTY. d. Failure by the AGENCY to correct noted deficiencies, as outlined in the written notice of corrective action, may result in the AGENCY being deemed in breach of the Contract terms. e. The AGENCY shall cooperate with the COUNTY on all reviews to ensure compliance with all applicable COUNTY guidelines and requirements for general fund recipients.
