Dental and Vision Insurance Coverage. A. All full-time regular and limited-term employees will be covered by the “Operating Engineers, Local 501, Security FundDental Program “Plan A”, and Vision Plan. B. Effective the first pay period after adoption of this 2015-18 MOU by the Board of Supervisors, the County will increase its contribution into the Operating Engineers, Local 501, Security Fund by $16.24 per pay period, for a total amount of fifty dollars and eighty-seven cents ($50.87) per pay period (exclusive of the third payday in any month) for each employee. C. To be eligible for the above payment, the employee must have been paid for at least seventy-two (72) regularly scheduled hours during the pay period for which the payment is being made. D. In the event the cost of maintaining the existing benefits exceeds $50.87 per pay period, the excess shall be paid by the employee through payroll deduction. E. Insurance coverages provided through the Security Fund with monies contributed by the County shall be made available by the Union to all eligible employees in the Representation Unit on an equal basis regardless of membership status. F. The Union shall indemnify and hold the County harmless from any claims or legal actions brought under this Section. G. IUOE will operate their insurance programs in accordance with applicable State and/or federal laws and regulations. Once each year, upon request of the County the IUOE Trustees shall provide the County with the following no later than December of each year: 1. Summary Annual Report of the Operating Engineers Local 501 Security Fund 2. Form 5500 3. Additionally, the County shall have the right to receive a copy of the full annual report, or any part thereof. The following is included in the report: a. an independent auditor’s report b. assets held for investment c. transactions in excess of 5% of plan assets and 4. A letter from the Certified Public Accountant verifying that all transactions of the trust have been reviewed and that all payments have been made consistent with the contractual agreements and the required tax returns have been filed in accordance with applicable laws.

H. The County and IUOE agree to a reopener of Section 3. (Dental and Vision Insurance Coverage) to discuss dental and vision coverage. This reopener shall not occur until the resolution of any and all civil or administrative proceedings resulting from the parties agreement in Section 3 including but not limited to, The Board of Trustees, in their capacities as Trustees of the Operating Engineers Local 501 Security Fund v. County of Orange Case no: 2:19-CV-09426.

Dental and Vision Insurance Coverage. APILO shall provide dental and vision insurance to all regular, full-time employees. Dental insurance coverage shall be provided through an employer-sponsored plan for all eligible employees. The coverage shall be substantially similar to the current dental insurance, which is provided by Delta Dental ($1,000 Annual Maximum) and administered by One Stop Payroll. Vision insurance coverage shall be provided through an employer-sponsored plan for all eligible employees. The coverage shall be substantially similar to the current vision insurance, which is provided by Eye Med/Delta Dental and administered by One Stop Payroll. Employees shall be permitted to obtain dental and/or vision coverage for a spouse, domestic partner, and/or dependents through API Legal Outreach’s available plans. The cost of such additional family members to an employee’s dental or vision plan is that employee’s sole responsibility and will not be paid for by the agency.

Dental and Vision Insurance Coverage. A. All HSOs shall be provided the PHT’s Dental and Vision coverage, on the same terms and conditions provided to all other PHT employees with regard to premiums, plan design, co-pays, deductibles, pharmacy, and co-insurance until December 30, 2021. B. Beginning on January 1, 2022, the premiums for the voluntary dental coverage will be increased by 10%. Beginning January 1, 2023, the premiums for the voluntary dental coverage will be increased by 10%.
