Description of Program. This Agreement is established to facilitate an exchange program at the partner institution with students, faculty and researchers from each institution (collectively, the “Exchange Program”). The student exchange program enables students from one university to take classes at the other university while working towards a degree at the university where he or she is formally enrolled (individually, the “Student Exchange”) and to encourage faculty members and researchers from each university to visit the other university and engage in certain activities (individually, the “Faculty/Researcher Exchange”). [Substitute the purpose of your program if different.] For purposes of this Agreement, with respect to the Student Exchange, the termHome University” shall mean the institution in which a student is formally enrolled as a degree candidate, and “Host University” shall mean the institution that has agreed to receive students from the Home University for a period of study. With respect to the Faculty/Researcher Exchange, as used herein, “Home University” shall mean the university where the faculty member and/or researcher is normally employed, and “Host University” shall mean the university that the faculty member and/or researcher is visiting.

The Center for Teacher Support (CTS) shall oversee teacher Peer Assistance, Peer Assistance and Review (PAR) and Induction as critical resource mechanisms that allow Consulting Teachers to assist Participating Teachers in gaining knowledge in subject matter and/or teaching strategies to support student growth. b. The Center for Teacher Support Panel will be responsible for administering the District’s Induction program to provide peer assistance that includes, but is not limited to: methods of delivering services, training, types of support offered, and recommendations to the school board on program design. c. The parties agree to jointly provide on-going support and awareness level training for PAR.
Description of Program. Evaluation Continued Consultation items: May 2015 - June 2016 The evaluation of the program will occur in two ways. To determine annual progress and program effectiveness, the district will analyze the student performance on approved pre- and post-assessments. The level of growth will be determined and reviewed by both the private school and public school officials. Parent, teacher and/or student surveys will be administered to determine the perceived impact. The surveys will be conducted at the close of the 2015-2016 SY. Results of the surveys will be shared with all parties.

Description of Program. The COUNTY’s program is for City Net to oversee and direct all community resource mobilization to enhance The Courtyard project and discourage duplicative service provision at the Civic Center. City Net will also participate with collaborative partners in The Courtyard to locate long term housing solutions for homeless neighbors.
Description of Program. The OPERATOR will be part of the County team to facilitate the successful rehabilitation and conversion of the FACILITY located at 0000 X. Xxxxxxx Place, Anaheim for a year round emergency shelter and multi‐service center. During Phase I, upon receipt of a Temporary Certificate of Occupancy and the Temporary License is executed, the OPERATOR will operate a year round emergency shelter for 100 residents providing shelter 365 days a year accessible to the shelter residents 24/7 until all construction to the FACILITY has been completed and the 20‐year lease is executed. In addition, Phase II will include a multi‐service center available to homeless clients with a range of on‐site programs and services provided by community partners. Access to bed availability will be handled through a reservation system. No walk‐ins for the PROGRAM or services will be permitted.
Description of Program. The CONTRACTOR will be part of the COUNTY team to facilitate the successful operations and administration of the PROGRAM located at 0000 X. Xxxxxxx Place, Anaheim for a year round emergency shelter and multi-service center. The CONTRACTOR will operate a year round emergency shelter for 200 residents providing shelter 365 days a year accessible to the shelter residents 24/7. In addition, the PROGRAM will include a multi- service center available to homeless clients with a range of on-site programs and services provided by community partners. Access to bed availability will be handled through a reservation system. No walk-ins for the PROGRAM or services will be permitted. 30 (thirty) of the 200 beds will be reserved for homeless clients of the County of Orange’s Health Care Agency Adult and Older Behavioral Health (HCA AOABH.) These clients are diagnosed with a serious and persistent mental illness and receive psychiatric and case management services from HCA. Reservations for these beds will be coordinated through HCA AOABH Residential Services Office (RSO.) Clients can enter either through HCA AOABH or directly. If clients are being referred by RSO a referral form and required Releases of Information will be faxed to CONTRACTOR staff. In the event a client arrives without prior notice facility staff will verify eligibility for the HCA bed with RSO, who will then request completion of the necessary forms. In the event all 30 RSO beds are not reserved by 5:00 PM they can be released for use by a non- HCA participant. CONTRACTOR staff will provide daily reports of bed occupancy and other relevant information related to client program participation and significant events, and a monthly summary to RSO. CONTRACTOR staff will also report the destination of exiting staff to RSO staff. HCA referrals will have access to all services available at the Year Round Emergency Shelter/Multi-Service Center. Medication will be stored, accessed, and dispensed in accordance with the guidelines established in the MOPSP and HCA requirements. CONTRACTOR staff will advise RSO staff in the event a participant is exhibiting signs of behavioral disturbance such as conflict with others, responding to internal stimuli, withdrawal, or other potential indicators of psychiatric decompensation. Onsite AOABH staff are to be notified in this event to assist with assessment.
