ELIGIBILITY STANDARDS. Employee eligibility standards for health care benefits shall be determined solely by the Board of Trustees and as permitted under existing law. The Trust is responsible for notifying newly eligible workers of their opportunity to enroll, enrolling eligible workers, providing open enrollment notifications and follow up to secure required applications/documentation, disenrolling ineligible workers and providing COBRA notifications and follow up. The Employer will provide the Trust with hours worked and other information needed by the Trust to determine eligibility, enroll eligible workers, and disenroll ineligible workers. The Employer will provide information on the Trust’s benefits to all employees during the onboarding process. The Trust shall determine the appropriate level of contribution, if any, by eligible home care workers. Ongoing costs for deduction of home care worker premiums for health care shall be paid by the Employer.

The Employer and the Union will work with the Trust to ensure, that in the future, the Trust has sole responsibility for notifying newly eligible workers of their opportunity to enroll, enrolling eligible workers, providing open enrollment notifications and follow up to secure required applications/documentation, disenrolling ineligible workers and providing COBRA notifications and follow up. The Employer will provide the Trust with hours worked and other information needed by the Trust to determine eligibility, enroll eligible workers, and disenroll ineligible workers. Employees may elect coverage for themselves, their spouses, domestic partners and other qualified dependents from any of the four (4) tiers (e.g., single, two-person, parent/child[ren] and family) in any of the four (4) plans offered by XXXX as delineated above. Employer contributions to a health reimbursement arrangement (HRA) or health savings account (HSA) will be made available in full regardless of the number of hours worked between 17.5 and full time, and not pro-rated.

The consent of the parent or guardian of a pupil who wishes to participate in interscholastic athletics must include an acknowledgment of the physical hazards that may be encountered in the sport. Notice of these eligibility requirements shall be given to students.

ELIGIBILITY STANDARDS. All bargaining unit members must meet the following eligibility standards: 1. The bargaining unit member must have exhausted fully all paid sick leaves prior to receiving Catastrophic Leave (excluding Extended Leave- Article 7.3 (difference pay). 2. Catastrophic Leave may only be used for an employee’s own catastrophic illness or injury. 3. Catastrophic Leave may not be used for personal necessity, normal pregnancy, cosmetic surgery, or mental stress. 4. Any bargaining unit member requesting Catastrophic Leave must have a medical doctor’s verification of his/her catastrophic illness or injury. 5. A bargaining unit member shall be eligible to apply for Catastrophic Leave after his/her one-year anniversary of employment with the District.
