External Quality Review. The Contractor shall undergo annual, external independent reviews of the quality, timeliness, and access to the services covered under this Contract, which are conducted pursuant to Subpart E of Part 438 of the Code of Federal Regulations. (42 C.F.R. §§ 438.350(a) and 438.320)
External Quality Review. The analysis and evaluation by an EQRO, or aggregated information in quality timeliness, and access to the health care services that the Contractor, or their contractors furnish to Medicaid beneficiaries.
External Quality Review. DCH will contract with an External Quality Review Organization (EQRO) to conduct annual, external, independent reviews of the Quality outcomes, timeliness of, and access to, the services covered in this Contract. The Contractor shall collaborate with DCH’s EQRO to develop studies, surveys and other analytic activities to assess the Quality of care and services provided to Members and to identify opportunities for CMO plan improvement. To facilitate this process the Contractor shall supply data, including but not limited to Claims data and Medical Records, to the EQRO.
External Quality Review. Section 1902(a)(30)(c) of Title XIX of the Social Security Act, requires the Commonwealth to acquire an independent external review body for the purpose of performing an annual review of the quality of services provided by an MCO under contract with the Commonwealth, including the evaluation of quality outcomes and timeliness of access to services. Requirements relating to the External Quality Review (EQR) are further defined and described under 42 CFR 433 and 438. The results of EQR are made available, upon request, to specified groups and to interested stakeholders. The Contractor shall provide information to the External Quality Review Organization EQRO as requested to fulfill the requirements of the mandatory and optional activities required in 42 CFR Parts 433 and 438. The Contractor shall cooperate and participate in EQR activities in accordance with protocols identified under 42 CFR 438, Subpart E. These protocols guide the independent external review of quality outcomes and timeliness of and access to services provided by a Contractor providing Medicaid services. In an effort to avoid duplication, the Department may also use, in place of such audit, information obtained about the Contractor from a Medicare or private accreditation review in accordance with 42 CFR 438.360.
External Quality Review. The state shall arrange for an annual, external independent review of the quality and outcomes, timeliness of, and access to covered services provided by the PIHP. The PIHP shall address the findings of the external review through its QAPIP. The PIHP must develop and implement performance improvement goals, objectives and activities in response to the external review findings as part of the PIHP's QAPIP. A description of the performance improvement goals, objectives and activities developed and implemented in response to the external review findings will be included in the PIHP's QAPIP and provided to the MDCH upon request. The MDCH may also require separate submission of an improvement plan specific to the findings of the external review.
External Quality Review. In addition to the following requirements, MCPs must participate in external quality review activities as outlined in OAC 5101:3-26-07.
External Quality Review. 35.6.2.1 The Contractor shall be available to participate in the EQRO’s onsite evaluation process during the Transition Period. The Contractor shall assist the EQRO with all reasonable requests including, but not limited to, providing samples of Grievances for the period covered by the Contract Term, by Service Region.
35.6.2.2 The HEDIS activity for calendar year 2013 shall be prepared by the new physical health service provider or providers. The Contractor shall provide all HEDIS data to the new physical health service provider or providers, as of the Termination Date of the Contract.
External Quality Review. SCDHHS will perform periodic medical audits through contractual arrangements to determine if the Contractor furnished quality and accessible health care to members in compliance with 42 CFR §438.358 (2006, as amended). SCDHHS may contract with an External Quality Review Organization to perform the periodic medical audits and external independent reviews. The Policy and Procedure Guide lists SCDHHS external quality assessment evaluation requirements.

On at least an annual basis, the Contractor will cooperate fully with any external evaluations and assessments of its performance authorized by the Division under this Agreement and conducted by the Division’s contracted External Quality Review Organization (EQRO) or other designee. Independent assessments will include, but not be limited to, any independent evaluation required or allowed by Federal or State statute or regulation. See Exhibit B, External Quality Review, of this Contract for additional requirements of the Contractor. The Contractor shall address any deficiencies or contract variances identified by the External Quality Review Organization (EQRO) expediently, on a schedule to be determined by the Division. The Division may issue sanctions for deficiencies or contract variances, which are not addressed to the satisfaction of the Division.
