Evaluation Requirements. 10.5.1 The Unit Member’s Evaluator shall be his/her Immediate Supervisor in the FCSS program.
10.5.2 The Evaluator’s role to assist the Unit Member may include, but not be limited to the following:
a. Specific recommendations for improvements;
b. County Office of Education assistance to implement such recommendations; and
c. Provision of available resources, without cost to the Unit Member, to be utilized to assist with improvements.
10.5.3 The Evaluator shall, at the time the evaluation conference commences, have the evaluation instrument completed (with the possible exception of specific recommendations(s) which may be mutually established if appropriate).
10.5.4 The Unit Member shall, at the beginning of the conference, have an opportunity to read his/her evaluation.
10.5.5 At the conclusion of the conference, the Unit Member and the Evaluator shall sign and date the evaluation form signifying only that the conference has taken place, not that the Unit Member concurs with the evaluation.
Evaluation Requirements. 6.5.1 Formal observations shall last at least thirty (30) minutes; shall be made known to the Unit Member at least two (2) working days prior to their occurrence; shall be followed by a conference with written feedback within five (5) working days of the
6.5.1.1 Probationary employees shall have no less than three (3) formal observations with one of those observations occurring by the close of the first semester.
6.5.1.2 Regular/Permanent employees shall have no less than two (2) formal observations. In cases where the Regular/Permanent Unit Member successfully achieves all standards, and by mutual consent of both Unit Member and the Evaluator, the number of observations may be reduced to one (1).
6.5.2 A Unit Member who receives a negative formal observation shall, upon request, be entitled to additional observations, observation conferences and written feedback as prescribed above.
6.5.3 In the case of a negative observation(s)/evaluation, the Evaluator shall take positive action to assist the Unit Member in correcting any cited deficiencies.
6.5.4 A negative observation may contribute in a Unit Member being denied permanent status.
6.5.5 The evaluation conference shall be held no later than May 1.
6.5.6 The Evaluator’s role to assist the Unit Member shall include, but not be limited to the following:
a. Specific recommendation for improvement;
b. County Office of Education assistance to implement such recommendation; and

6.5.7 In reference to Regular/Permanent employees, in preparing the final evaluation form for placement in the Unit Member’s personnel file, the Evaluator shall rely primarily upon data collected through observations and evaluation conferences. Any deficiencies that have been brought to the attention of the Unit Member, and subsequently corrected, or demonstrated notable progress, may not be included in the final evaluation form. Unsubstantiated statements shall not be included in the evaluation.
Evaluation Requirements. The School Board, pursuant to its constitutional duty to supervise the public schools in the school division, will annually receive a presentation from the Charter School which will include the Charter School’s progress toward the academic measures stated herein and an updated budget. The School Board will also review and consider the audit referenced above.

The evaluation design shall discuss the State’s process for obtaining an independent entity to conduct the evaluation, including a description of the qualifications the entity must possess, how the State will assure no conflict of interest, and a budget for evaluation activities. Evaluation Design. The Evaluation Design shall include the following core components to be approved by CMS: Research questions and hypotheses: This includes a statement of the specific research questions and testable hypotheses that address the goals of the demonstration. At a minimum, the research questions shall address the goals of improving access, reducing churning, improving quality of care thereby leading to enhanced health outcomes, and lowering costs. The research questions will have appropriate comparison groups and may be studied in a time series. The analyses of these research questions will provide the basis for a robust assessment of cost effectiveness. Evaluation Requirements. A. Each completed evaluation will result in the assignment of a teacher effectiveness rating of Accomplished, Skilled, Developing, or Ineffective. The teacher effectiveness rating shall be derived from a summative evaluation where fifty percent (50%) of the overall evaluation is based on a teacher’s performance rating as provided in this Agreement, and fifty percent (50%) of the overall evaluation is based upon student growth measures as provided for in this Agreement.
B. Every teacher shall be evaluated every year. The district retains discretion to remove a teacher from the evaluation cycle. Generally, a teacher may not be evaluated if one of the following applies:
1. A teacher who has attained a summative rating of skilled, is not in the first three years of employment, and maintains a student growth rating of average may be evaluated every other year. In the event a skill teacher is going to be evaluated, the teacher will be notified of the reason in writing no later than September 1.
2. A teacher who has attained a summative rating of accomplished, is not in the first three years of employment, and maintains a student growth rating of average may be evaluated every third year. In the event an accomplished teacher is going to be evaluated, the teacher will be notified of the reason in writing no later than September 1.
3. A teacher who is on leave for 50% or more of the school year.
4. A teacher has submitted an irrevocable written notice of retirement on or before December 1 of the school year in which they will retire.
C. Teachers who are not formally evaluated must establish growth plans, develop student learning objectives, and will have one observation with a post conference to provide feedback. This observation shall be a minimum of 30 minutes.

D. Professional Growth and Improvement Plan

2. Teachers who have attained a final holistic rating of skilled will develop a professional growth plan in collaboration with the assigned evaluator.
3. Teachers who have received a final holistic rating of “Developing” will develop a professional growth plan that is guided by their assigned evaluator
4. Teachers who have received a final holistic rating of “Ineffective” will be placed on an improvement plan by their assigned evaluator.
5. Accomplished teachers will choose two (2) goals for professional growth plans. Teachers with a summative rating of Skilled or Developing will set two (2) goals in collaboration with their evaluator based upon the descriptions above.

There shall be filed with the Human Resources Department an annual evaluation for each permanent employee evaluating their performance in their particular position for the period covered in the written evaluation. At the discretion of the immediate supervisor, unit members with five (5) or more years with the District may be evaluated once every two (2) years if the last two (2) evaluations have been overall outstanding.

b. The state shall also conduct an evaluation pursuant to STC 27 which shall include an investigation of the impact of providing Medicaid reimbursement for IMD services on the following outcomes among beneficiaries in need of acute mental health or substance use disorder treatment:
i. Emergency room utilization for consequences of substance use disorders including opioid overdoses;
ii. Access to acute inpatient treatment and residential treatment for substance use disorders;
iii. Lengths of stay in acute inpatient and residential settings for treatment for treatment of substance use disorder;
iv. Access to acute inpatient and residential treatment for substance use disorders;
v. Quality of substance use disorder treatment including medication assisted treatment;
vi. Quality of discharge planning in making effective linkages to community- based care;
vii. Readmissions to the same level of care or higher;
viii. Cost of treatment for substance use disorder conditions;
ix. Overall cost of care for individuals with substance use disorders including co-morbid physical and mental health conditions;
x. Opioid prescribing patterns; and,
xi. Drug overdose deaths.
