HEALTH AND LIFE INSURANCE PROGRAM. The parties agree that the H&W contribution will be $4.02 per paid hour. Effective 1 December 2019 the rate will be $4.25 per paid hours. The Company shall be entitled to a credit against the Supplemented DOL Fringe Benefit Rate in the amount of the cost of furnishing fringe benefits under this Agreement. The Supplemented DOL Fringe Benefit Rate will be provided only on paid hours up to 40 hours per week. Any monies remaining after health coverage premiums have been paid shall be directed into the employees check. Employees may elect to place all or portions of their residual Health and Welfare into the Companies 401k plan. December 1st of each year employees may complete a residual H&W election form on a pre-tax basis to elect this benefit. The Company will offer Insurance Plans which will include the following benefits at a minimum: • Medical insurance • Vision • Dental • Accidental Death & DismembermentLong Term Disability • Short-Term Disability • Life Insurance The first month after thirty (30) days of employment, employees covered by this agreement will be eligible for insurance coverage under the Company's Insurance benefit plans. Types of coverage, premium rates and benefits will be communicated to the employees and the Union during open enrollment on an annual basis. Both parties agree that the employee paid portion of the monthly premiums, as stated below and the insurance benefits will be subject to change each year. Upon verification of other qualified group medical care coverage, employees have the option of opting out of the Company plans.

HEALTH AND LIFE INSURANCE PROGRAM. During the term of this Agreement, health and life insurance programs shall be provided to all eligible employees covered by this Agreement in accordance with Illinois State Employees Group Insurance Act of 1971, 5 ILCS 375 et. seq. The parties agree to accept the terms and conditions of life and health programs as provided by the Department of Central Management Services at a statewide level intended to apply to state universities. When provided the opportunity the University will advocate on behalf of the healthcare needs for all employees to include male, female and transgender employees.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • Health Insurance Plan (Excluding Summer Students Regardless of Wage Schedule Paid From) These employees shall be considered as a group in order that they may apply to participate in the Supplementary Plan and the Extended Health Benefit Plan at group rates. One hundred percent (l00%) of all premiums will be paid by the employees. The Company will pay one hundred percent (l00%) of the Ontario Health Insurance Plan premium for temporary employees who have four months' accumulated service.

  • Hospitals of Ontario Voluntary Life Insurance Plan The Hospital also agrees to make the Hospitals of Ontario Voluntary Life Insurance Plan (HOOVLIP) available to the nurses subject to the provisions of HOOVLIP at no cost to the Hospital.

  • Ontario Health Insurance Plan The parties recognize that the method of funding OHIP has been changed from an individually paid premium to a system funded by an employer paid payroll tax. If the government, at any time in the future, reverts to an individually paid premium for health insurance, the parties agree that the Colleges will resume paying 100% of the billed premium for employees.

  • Insurance Plan 19.01 The Employer agrees to contribute the indicated percentage of the premium cost of the following group plans for full-time employees (and their families where applicable) who have completed their probationary period.

  • Life Insurance No portion of your IRA may be invested in life insurance contracts.

  • Insurance Program An eligible employee may waive rights to participate in either single or family coverage. If an employee waives this benefit, such employee may not revoke the waiver until the next open enrollment period and may be accepted only after medical review by the insurance provider.
