Health Care Reimbursement Account. Eligible employees will have the option to participate in a pre-tax Health Care Reimbursement Account as defined by the Internal Revenue Service and as outlined in the Educated Choices Workbook.
Health Care Reimbursement Account. The County will administer a Health Care Reimbursement Account that will allow eligible employees the opportunity to allocate a specific amount of biweekly pre-tax salary into the employee’s health care reimbursement account to pay for health care expenses as permitted by state and federal law, regulations and guidelines and as permitted by the County’s Section 125 Plan document.

Health Care Reimbursement Account. A. The City of Canton agrees to participate in the Security Benefit Group Healthcare Reimbursement Account (HRA) Plan for Public Sector Labor Association Employees (The Plan) in accordance with the terms and conditions of this Plan’s Participation Agreement. The parties hereto designate Financial Network of America LTD, or its successors appointed in accordance with the Plan and Trust documents, to serve as the Plan Administrator for the Plan. The parties hereto; may designate another Plan Administrator during the term of this Agreement by mutual consent. The Employer agrees to contribute to the Plan on behalf of the following employees:

B. Employees may choose to direct the money upon separation of service or retiring from the City of Canton to the Security Benefit Group Healthcare Reimbursement Account (HRA) Plan for the Public Sector Labor Association Employee. If the member executes the appropriate forms to authorize the transaction, the City will pay the money directly into the Security Benefit Group Healthcare Reimbursement Account (HRA) Plan for Public Sector Labor Association Employee of the member’s choosing. Employee shall submit in writing to Human Resources and the Auditor sixty (60) days in advance a request that said member’s pay be distributed by an amount or percentage into the HRA plan.
Health Care Reimbursement Account. The administrator will have the option to participate in a pre-tax Health Care Reimbursement Account as defined by the Internal Revenue Service and as outlined in the Educated Choices Workbook.
Health Care Reimbursement Account. Allina Health agrees to make Health Care Reimbursement Account employer contributions in the amount of $500 for any nurse with a designed FTE of 0.1 or greater who is not enrolled in the Select Health Savings Plan or Basic Health Savings Plan during the 2017 plan year. Such employer contributions will reduce the maximum employee contributions that may be made under such Account. The employer contributions will deemed to be made to the general purpose Health Care Reimbursement Account (rather than to the Limited Purpose Health Care Reimbursement Account), unless such nurse provides notice within 30 days of receipt of such employer contributions that such nurse prefers the contributions to be made to the Limited Purpose Health Care Reimbursement Account in order to preserve health savings account eligibility.
Health Care Reimbursement Account. Contributions to fund a Health Care Reimbursement Account are permitted (Section 4.02) (If "No", questions regarding Health Care Reimbursement Accounts are disregarded.): 11b. HSA Account. Contributions to fund an HSA Account are permitted (Section 4.08):
Health Care Reimbursement Account. With respect to each LFCM Employee who as of the date hereof participates in a healthcare flexible spending reimbursement account (“Health Care Reimbursement Account”) under the Lazard Group Cafeteria Plan, in accordance with the procedures set forth in this paragraph, LFCM shall cause the LFCM Cafeteria Plan to accept a spin-off/transfer of the Health Care Reimbursement Account for such LFCM Employee from the Lazard Group Cafeteria Plan and shall honor and continue through the end of the 2005 calendar year the elections made by each such LFCM Employee in respect of such Health Care Reimbursement Account that are in effect immediately prior to the date hereof. LFCM shall take all actions it deems necessary or appropriate to effectuate the foregoing and to provide the LFCM Employees with Health Care Reimbursement Accounts under the LFCM Cafeteria Plan for the remainder of the 2005 calendar year on terms and conditions that are substantially similar to the terms and conditions applicable to the Health Care Reimbursement Accounts under the Lazard Group Cafeteria Plan as of the date hereof. As soon as practicable following the date on which the Flex Account Statement (as defined below) becomes final and binding in accordance with this paragraph, (a) Lazard Group shall cause to be paid to LFCM cash in an amount equal to the excess, if any, of (i) the aggregate accumulated contributions by the LFCM Employees to the Health Care Reimbursement Accounts under the Lazard Group Cafeteria Plan made prior to the Close of the date hereof during the 2005 calendar year, over (ii) the aggregate Health Care Reimbursement Account reimbursement payments made to the LFCM Employees prior to the Close of the date hereof for the 2005 calendar year from such accounts, and (b) LFCM shall cause to be paid to Lazard Group cash in an amount equal to the excess, if any, of (i) the aggregate Health Care Reimbursement Account reimbursement payments made to the LFCM Employees prior to the date hereof for the 2005 calendar year from the Health Care Reimbursement Accounts under the Lazard Group Cafeteria Plan over (ii) the aggregate accumulated contributions to such Health Care Reimbursement Accounts made by such LFCM Employees prior to the Close of the date hereof during the 2005 calendar year. From and after the Close of the date hereof, LFCM shall cause the LFCM Cafeteria Plan to assume and be solely responsible for all claims incurred in respect of the Health Care Reimbursement Accounts ...
Health Care Reimbursement Account a. The District agrees to offer a health care reimbursement account to allow active employees to set aside money from each paycheck, tax-free, to reimburse themselves for certain health care expenses incurred during the year not paid by any insurance plan.
b. Eligible expenses include amounts employees pay out-of-pocket for most necessary treatments or services to prevent or improve a medical condition, such as health care plan deductibles and co-payments and charges for various health care services and supplies not covered or partially covered under health benefit plans.
c. Those who decide to enroll in the health care reimbursement account may contribute up to the amount allowable by the IRS of pre-tax salary each calendar year, distributed evenly between each pay period. Employees may not make any changes to their elected amount during the year unless certain changes in family status take place, such as the birth of a child or change in employment status of the employee or employee's spouse, which would affect the use of health care.
d. Any money left in the health care reimbursement account at the end of the calendar year may be subject to carry over to the following calendar year according to applicable IRS rules; employees have until March 31 of the following year to submit claims for eligible expenses incurred through December 31 of the year they participated in the account.
e. During the month of December of each year, employees may opt-out, re-enroll and/or amend amounts to be contributed for the next calendar year, with amendments effective January 1 of the following year.
