Medical Insurance Plans. The City will offer the Xxxxxx Permanente HMO plan (“Kaiser HMO”) and the Blue Shield of California HMO plan (“Blue Shield HMO”) to eligible Unit employees. All plans offered by the City are subject to carrier availability.

Medical Insurance Plans. The indemnification plan shall be substantially comparable to the medical insurance plan currently offered by the City through Pacific Source Health Plans and shall include limited chiropractic care and a $15.00 co-pay. Employees may, at their option, select the City’s HMO plan offered through Xxxxxx Permanente.
Medical Insurance Plans. The City will provide employees with the following plans, each as offered through the Association of Washington Cities Benefit Trust AWC Regence HealthFirst High Deductible Health Plan, Xxxxxx Permanente High Deductible Health Plan, AWC Regence HealthFirst $250 plan, or Xxxxxx Permanente Access PPO 12.2 Premiums.
Medical Insurance Plans. 1. Effective the first of the month following the effective date of the Agreement, or as soon thereafter as the City can complete an appropriate transition, the existing medical and hospital insurance plans provided by the City will be updated with the Regence PPO Plan (PPO), the Regence High Deductible Health Plan/Health Savings Account (HDHP/HSA) Plan and Xxxxxx Permanente HMO Plan as described in Appendix C. The City will continue to provide medical insurance to employees and their eligible dependents through the plans described in Appendix C.

Medical Insurance Plans. The Preferred Provider Organization (PPO) plan shall be substantially comparable to the medical insurance plan currently offered by the City through Aetna Open Choice PPO Plan. Employees may, at their option, select the City’s HMO plan offered through Xxxxxx Permanente.
Medical Insurance Plans. Effective August 1, 1996 the maximum monthly City medical contribution shall be $455.00 toward any of the PERS health plans. Thereafter, in any year that the full- family premium for the Kaiser North or Aetna Health Plan effective August 1 increases and exceeds $455, the City's medical premium contribution shall increase by the change in the San Francisco-Oakland-San Xxxx Area Consumer Price Index for Urban Wage Earners and Consumers from April of the prior calendar year to April of the current year.

Medical Insurance Plans. Your medical insurance is typically used if you have an eye problem or eye disease or if any medical condition is present that causes eye problems. Some common conditions for which we may bill your medical insurance include:  Redness  Eye floaters  Allergies  Dryness  Infections  Monitoring cataract development  Examination of patients having diabetes  Examination of patients using medications with potential eye side effects, such as steroid medications, arthritis medications, etc.  Patients that are at high risk for glaucoma development  Patients with macular degeneration Unfortunately, due to policies of the insurance companies, you cannot use your vision and medical insurance for a joint exam on the same day. The only exception to this are those patients who have VSP Vision insurance. Below are two possible alternatives.
