Medical Reimbursement. Under the terms of this Agreement, State Arresting Agencies will pay medical costs of inmates confined in the county detention center who are the responsibility of the State Arresting Agency. Medical costs shall be based on currently approved Medicaid reimbursement rates. State Arresting Agency will not reimburse expenses exceeding these rates. The State Arresting Agency agrees to provide direct payment to the medical service provider, if requested to do so by County.
1.) The determination to provide an inmate with non-emergency medication, medical services, or hospitalization shall be at the discretion of the detention center administrator.
2.) Prior to authorizing non-emergency medical services for inmates whose medical costs are the responsibility of the Department of Corrections (DOC), County shall contact the DOC Health Services Bureau Chief at (406) 846- 1320 ext. 2254.
3.) County agrees to provide inmates with emergency medical treatment in accordance with acceptable standards of practice.
4.) In the event an inmate requires hospitalization and the resulting medical costs are the responsibility of a State Arresting Agency, the State Arresting Agency agrees to reimburse County for costs associated with providing on-site security, including wages, employer contributions, and related incidental costs. County agrees to notify the State Arresting Agency of said hospitalizations within 24-hours of occurrence.
Medical Reimbursement. This program permits employees to pay for common out-of-pocket medical expenses (not covered by insurance) such as deductibles, co-pays, and vision and dental care with pre-tax dollars.
Medical Reimbursement. The Company shall reimburse Employee for medical expenses incurred by Employee for himself or his immediate family which are not covered expenses pursuant to the health insurance policies provided by Company pursuant to paragraph (b), above, in an amount not to exceed Three Thousand Dollars ($3,000) per annum.
Medical Reimbursement. EMPLOYER has established a medical reimbursement plan for certain of its key executive officers including EMPLOYEE. During the Term, EMPLOYER shall maintain such plan and shall pay all premiums in connection with such medical reimbursement plan.
Medical Reimbursement. The Employee shall be entitled to receive reimbursement for documented medical expenses of the Employee and her dependents not otherwise covered by the Company’s medical plan and long term care and disability insurance coverage for the Employee.
Medical Reimbursement. The Employer will match employee contributions to the Section 125 Medical Reimbursement plan at the rate of $0.50 per $1.00 employee contribution up to a maximum Employer contribution of $100 per year. This provision shall become effective at the time of signing of the Agreement.
Medical Reimbursement. A. Bargaining unit members who are presently covered under another health insurance program (i.e., through spouse’s employer) and, therefore, not electing health insurance coverage may apply for medical reimbursement plan as follows:
1. If husband and wife are both members of this bargaining unit, one shall elect health insurance coverage and the other shall elect the medical reimbursement plan in lieu of the Health Insurance. Effective July 1, 2016 if member and spouse/other eligible adult are both FSU employees, they cannot receive both health coverage and opt-out payments.
Medical Reimbursement. The Board will provide to each staff member the necessary forms for medical reimbursement as well as an instruction sheet. Employees shall then submit such forms directly to the medical insurance carrier. All requirements relating to the completeness and/or correctness of the forms shall be the sole responsibility of the employee. The Board Business Office will not become involved in any manner at any stage of the processing of such forms.
Medical Reimbursement. Employees working less than thirty hours per week who are not eligible for a health insurance benefit, will receive as a medical reimbursement in the amount of $150. This reimbursement check will be paid at the end of each school year. Employees working more than thirty hours per week who waive their health insurance benefit are not eligible for this payment.
Medical Reimbursement. Upon proof of payment, Sun agrees to reimburse Executive out of pocket costs for maintaining his current medical insurance. Said costs are estimated to be $990 per month. During this time Executive will not participate in Sun's health plan. Executive agrees that he will provide such proof of payment to Sun promptly after Executive incurs each such cost.
