Hospital Insurance. The Hospital agrees to pay seventy-five (75%) of the billed premiums towards coverage of eligible employees in the active employ of the Hospital under the Liberty Health Plan or comparable coverage with another carrier.
Hospital Insurance. The Employer shall contribute one hundred percent (100%) towards the billed premium for standard xxxx coverage under the Ontario Health Insurance Plan.
Hospital Insurance. Participant or Preferred Provider who is a hospital or ambulatory service center, at its sole cost and expense, shall procure and maintain such policies of insurance as are necessary to insure Participant or Preferred Provider and all Providers, employees, agents and representatives with coverage limits of not less than one million dollars ($1,000,000) per occurrence, three million dollars ($3,000,000) in annual aggregate, and five million dollars ($5,000,000) excess coverage in the performance of any act relating to this Agreement. Upon request, Participant or Preferred Provider agrees to submit to ACO a certificate of insurance as evidence of such coverage. In the event any such liability policy is a "claims made" policy, Participant or Preferred Provider will purchase a "tail" policy, effective upon the termination of the primary policy, or obtain replacement coverage which insures for prior acts, insuring for losses arising from occurrences during the term of this Agreement, which tail policy or prior acts coverage shall have the same policy limits as the primary policy and shall extend the claims reporting period for the longest period for which coverage is available.
Hospital Insurance. The Milford School District will participate fully in the State subsidized plan of Medical/Hospital insurance as authorized in 29 Delaware Code, Chapter 52. Since 29 Delaware Code, Chapter 52 does not provide basic medical/hospital insurance coverage for personnel who have not completed at least three months of employment for the State of Delaware, the Board agrees to subsidize said medical/hospital insurance coverage for these full-time Employees from local funds in an amount not to exceed identical coverage provided for experienced Employees by the state. Coverage from local funds will terminate as soon as the Employee qualifies and is enrolled in the state subsidized program. Any first year full-time Employee who qualifies for participation in the State Medical/Hospital Insurance Program, except for the three-month (3) experience clause, is eligible for inclusion in the local program.

Hospital Insurance. Effective as of January 1, 1990 the Employer Health Tax replaced OHIP premiums. Should the premium method be re-enacted at any time it is agreed that the Employer shall be responsible for paying the legislated amount applicable to hospital premiums.
Hospital Insurance. Current practices will prevail for the duration of this Agreement, except that any changes in medical or hospital insurance plans, including the premium payable by employees, applicable to the majority of those employed in the Public Service for whom the Treasury Board is the employer, will during the life of this Agreement be applicable to the employees under this Agreement.
Hospital Insurance. Full-time employees who were employed by the District prior to July 1, 2009, who retire prior to the age of Medicare eligibility and who meet the conditions established by Article XII, Subd. 3, 3.1, may individually elect to continue participation in the group hospitalization plan established by Article VI, Section 3 at the employee's personal cost until Medicare eligibility. Monthly premiums shall be paid by the retired employee one month in advance at the School District's Office.
Subd. 1 School Board Contributions The School Board will contribute according to Article XV, Section D of the teachers’ contract, single coverage only, up to a maximum of eight years or Medicare eligibility whichever is first according to the following formula.
1.1 Four years of health insurance upon retirement.
1.2 One year of health insurance if the employee has served the district for twenty (20) years.
1.3 One year of health insurance if the employee has served the district for twenty-three (23) years.
1.4 One year of health insurance if the employee has served the district for twenty-five (25) years and had thirty (30) accumulated sick days.
1.5 One year of health insurance if the employee has served the district for thirty (30) years and has sixty (60) accumulated sick days. Eligibility: To be eligible for the early retirement incentive program established by Sections 1 and 2 an employee must be full-time (normal work week of 30 hours or more).
Section 1. Definition:
Hospital Insurance. A. Employees hired prior to December 1, 1991. The City shall provide for employees and eligible members of- employee's family hired prior to 12-1-91 Blue Cross MVF-2 Master Medical with Mandatory Second Opinion, Drug and Reciprocity Riders. The cost shall be sus- tained by the City. Retired employees who were hired prior to 12-1-91 shall continue to be covered by this plan, cost sustained by the City, until the retired employee, either active or retired, become deceased, said employee's spouse and eligible dependents under the plan shall continue to be covered by said insurance, provided said spouse remains unmarried. Retired employees who obtain employment from an employer who' provides HospitaJ. Insurance shall not be covered by the City's Hospital Insurance for duration of such employment.
B. Employees hired after December 1, 1991. All employees and eligible members of employee's family hired after 12-1-91 will be covered by an HMO with the 3ame coverage as the Blue Cross/Blue Shield Plan, cost sustained by the City. The City may at its option offer one or more plans. Employees who were hired af:9f 12-1-91 who wish to upgrade to the Blue Cross/Blue Shield Plan may do so durin~ enrollment period with not less than a ninety (90) day written notice to payroll 'J(fice at the employee's own expense for the differential in premium cost (if any) via payroll deduction or direct payment to the City. Retired employees who were hired after 12-1-91 shall be covered by the HMO plan with the same coverage as the Blue Cross/Blue Shield Plan, cost sustained by the City, until the retired employee, either active or retired, becomes deceased. Said employee's spouse and eligible dependents under the plan shall continue to be covered, provided said spouse remains unmarried. If said retired employee and/or spouse intends to move out of the area covered by the HMO, he/she must give the city a minimum of 90 days prior notice of the location he or she intends to move to, and the city shall arrange for coverage by another HlVIOor other plan covering that area which provides the same or better coverage. If no said HMO is available, then the retired employee and or spouse will be covered by the above references Be/aS plan for retirees.
