Health Insurance Benefit. The Board shall pay, in addition to salary, a fringe benefit for teachers. The Board agrees to pay the cost of the lowest tier single plan of the district provided health insurance to be applied towards a district health insurance plan. Said benefit will not exceed $510.00 per month. The plan will cover all certified/licensed teachers employed at least .5 FTE Certified/licensed teachers who began employment prior to August 11, 1998, and choose not to enroll in the district’s health insurance plan, will receive a benefit of $64 per month as cash. This is a fixed cash amount; it shall not increase even if the Board’s contribution to the insurance plan increases. Teachers hired after August 11, 1998, must use their benefit for the purpose of purchasing the district’s health insurance plan. New employees who do not wish to purchase the health insurance plan will forfeit the benefit amount. Should those who choose the cash option later decide to join the district’s health plan, they shall forfeit the cash since it will be applied to the insurance plan. Furthermore, this is a one-way decision, in that a person moving from a cash option to an insurance fringe cannot later go back to the cash option.

Health Insurance Benefit. 1. A unit member at retirement, with a minimum of ten (10) consecutive years of service with the Jamesville-Xxxxxx School District, may continue in the Health Insurance Program provided that he/she actually and officially retires as per the requirements established by the N.Y.S.
Health Insurance Benefit. Employees covered by this agreement working a minimum of thirty (30) hours per week on a regularly scheduled basis will be eligible for a County sponsored health insurance plan. 8.8.1 Regular employees who work a minimum of thirty (30) hours per week on a regularly scheduled basis will be eligible for a County sponsored health insurance benefit the first day of the month following 60 days of employment. Regular employees who wish to enroll in the County’s benefit plan before completing 60 days of employment may do so by paying one hundred percent (100%) of the plan cost. 8.8.2 It is the responsibility of the employee to notify his/her Department Head if he/she wishes to enroll in the plan or he/she may take such an election in the thirty (30) day period prior to the insurance contract renewal date. 8.8.3 The employee may elect to participate in a single person, two person or family plan. A tax deferred option commonly known as Section 125 of the IRS Code is available for the employee’s share when the County pays its share. 8.8.4 Coös County will pay 85% of the premium for a single person plan and 80% of the premium for a two-person or a family plan. Effective July 1, 2019 to January 31, 2020, the plan referred to the New Hampshire Interlocal Trust “HMO $20/$40/$1,000” plan, and a $0/$10/$30/$50 prescription drug plan. Effective February 1, 2020 the plan referred to HealthTrust AB15IPDED plan and retail 10/25/40 Mail 10/40/70 prescription plan. 8.8.5 The County’s monthly payments will cease in the month that an employee terminates employment. Any payroll deductions, which have been taken for the subsequent month, will be refunded to the employee. 8.8.6 Employees on unpaid leave of absence will be required to pay one hundred percent (100%) of their health insurance costs if such leave exceeds thirty (30) calendar days unless an employee is on a paid Family and Medical Leave. In the event of unpaid FMLA leave, an employee shall pay the County the employee’s share of any medical insurance premium once per month in advance of the first day of each month. 8.8.7 Employees on worker’s compensation leave will continue to receive health insurance in accordance with Section 8.8.4 for a period of six (6) months from their last day of active employment after which they will be required to pay one hundred percent (100%) of their health insurance costs. 8.8.8 It is specifically agreed that the County may, in its sole discretion, obtain health insurance from a source of i...
Health Insurance Benefit. Effective January 1, 2019, the University will allow full-time bargaining unit members to select one of three health insurance plans as set forth herein: 1. The "90/70 PPO" comprehensive medical insurance plan with deductibles, co-insurance, out-of-pocket limitations and other significant plan features as set forth in Attachment A hereto. Effective January 1, 2020, this plan will cease to be available. 2. The "85/60 PPO" comprehensive medical insurance plan with deductibles, co-insurance, out-of-pocket limitations and other significant plan features as set forth in Schedule A hereto. 3. A High Deductible Health Plan (HDHP), and a Health Savings Account (HSA) option, with deductibles, co-insurance, out-of-pocket limitations and other significant plan features as set forth in Schedule A Plan Comparison Chart. 4. Contributions will be based upon the employee’s salary on a schedule as depicted by the sample on Attachment A attached.

A surviving spouse may continue coverage in accordance with die provisions of COBRA.

Health Insurance Benefit. All AmeriCorps Members serving in a full-time capacity, must be covered by health insurance, and are eligible to receive health insurance coverage through their AmeriCorps program. There will be no monthly premium charge to Members, however, a co-pay, deductibles, and any other payments as outlined in the health insurance policy, will be the responsibility of the Member. The $100 deductible charge for the health plan and the $25 deductible charge for the dental plan will be reimbursed by CTEP if the Member presents a copy of the bill to CTEP staff showing the deductible costs.
