Member Notification. To the extent required by law, any indemnification of or advance of expenses to a Member or Manager in accordance with this Article shall be reported in writing to the Members with or before the notice or waiver of notice of the next Members' meeting or with or before the next submission to Members of a consent to action without a meeting and, in any case, within the twelve month period immediately following the date of the indemnification or advance.
Member Notification. The University agrees that in the interest of informing members of the University community about current services and policies, it will:
1. distribute employee assistance program brochures annually to all members of the bargaining unit (including a copy sent each year to the Association); and
2. include, in information available to members on the University’s webpage, entries for "day care" and "elder care" and other related services offered (for example, "drugs and alcohol") with cross references to "benefits" and "employee assistance program."
Member Notification. The Contractor shall send a written notice within fifteen (15) calendar days of notice or issuance of termination of a Provider to Members who receive primary care from the Provider, who are treated on a regular basis from the Provider, or who are affected by the loss of the Provider for other reasons. The written notice shall include information about selecting a new Provider, and a date after which Members that are undergoing an active course of t reatment cannot use the terminated Provider. The Contractor shall receive the Division prior approval for Member notices.
Member Notification. To the extent required by law, any indemnification of or advance of expenses to a Person in accordance with this Article X shall be reported in writing to the Member within ten (10) days immediately following the date of the indemnification or advance.
Member Notification. The Contractor shall provide Members written notice of an Adverse Benefit Determination in writing consistent with 42 C.F.R Part 438.404 and 438.10. The notice must explain the following:
Member Notification. To the extent discretionary to the Company, the Managing Member shall approve or disapprove of indemnification or advancement of expenses under this Article VII. Any indemnification of or advance of expenses to any Person entitled or authorized to be indemnified under this Article VII shall be reported in writing to the Managing Member within the twelve (12) month period immediately following the date the indemnification or advance was made; provided, that no failure to comply with the notification provisions of this Section 7.8 shall operate to deprive a Person of any indemnification or advancement of expenses to which such Person would otherwise be entitled.
Member Notification. Any indemnification of or advance of expenses to any Person entitled to be indemnified under this Article VIII shall be reported in writing to the Members with or before the notice or waiver of notice of the next Members' meeting or with or before the next submission to Members of a consent to action without a meeting and, in any case, within the 12 month period immediately following the date the indemnification or advance was made.
Member Notification. The District shall make a reasonable effort to notify the Federation treasurer within five days if any member resigns or is terminated and such action is approved by the Board of Trustees. Within thirty (30) calendar days of hire, the District shall electronically provide the Federation with the following information for any newly hired bargaining unit employees. The District shall make a reasonable effort to notify the Federation treasurer within thirty (30) days if any member resigns or is terminated after such action is approved by the Board of Trustees.

j. Hire date The Federation agrees to indemnify the District in the event of a Federation data breach in which the information provided by the District is compromised. The District shall be notified by the Federation as soon as feasible regarding any such data breach.
Member Notification. The Contractor shall provide Members written notice of an Adverse Benefit Determination in writing consistent with 42 C.F.R Part 438.404 and 438.10. The notice must explain the following:
1. The Adverse Benefit Determination the Contractor has made or intends to make;
2. The reasons for the Adverse Benefit Determination, including the right of the Member to be provided upon request and free of charge, reasonable access to and copies of all documents, records, and other information relevant to the Member’s Adverse Benefit Determination. Such information includes medical necessity criteria, and any processes, strategies, or evidentiary standards used in setting coverage limits;
3. The Member's right to request an appeal of the Contractor’s adverse benefit determination, including information on exhausting the Contractor’s one level of appeal described at § 438.402(b) and the right to request a State Fair Hearing;
4. The procedures for exercising the rights specified in this subsection;
5. The circumstances under which an appeal process can be expedited and how to request an expedited appeal; and
6. The Member's right to have benefits continue pending resolution of the appeal, how to request that benefits be continued, and the circumstances under which the Member may be required to pay the costs of these services. The notice(s) as described within this subsection must be mailed within the following timeframes:
1. For termination, suspension, or reduction of previously authorized Medicaid-covered services, at least ten (10) calendar days prior to the date of the Adverse Benefit Determination;
2. For denial of payment, including denial of payment that may result in Member financial liability, at the time of any action affecting the claim;
3. For standard service authorization decisions that deny or limit services, within ten (10) calendar days of the date of the Adverse Benefit Determination.
Member Notification. To the extent discretionary to the Company, the Board by Majority Consent shall approve or disapprove of indemnification or advancement of expenses under this Article VII. Any indemnification of or advance of expenses to any Person entitled or authorized to be indemnified under this Article VII shall be reported in writing to the Board with or before the notice or waiver of notice of the next Board meeting or with or before the next submission to the Board of a consent to action without a meeting and, in any case, within the twelve (12) month period immediately following the date the indemnification or advance was made.
