Notification of Employer. When an employee is unable to report for work, he or she shall notify his or her immediate supervisor or other designated person prior to the time he is scheduled to report to work on each day of absence.
Notification of Employer. An employee who is unable to perform his or her work due to incapacity for work must notify the employer accordingly no later than one hour before the start of the shift on the first day of incapacity for work, except if the employee cannot reasonably be required to do so, in which case the employer must be notified of the incapacity for work as soon as possible.

Every employee shall notify or cause someone on his or her behalf to notify the Department or Branch without delay and whenever possible prior to the start of his or her shift if he or she is unable to report for any reason outlined in Article 22-1. Employees attending pre-scheduled medical appointments shall provide at least 24 hours of advance notice to their Department or Branch.

Notification of Employer. An employee on bereavement leave shall inform his/her supervisor of the fact and reason therefore as soon as possible. Failure to do so within a reasonable time may be cause for denial of bereavement leave with pay for the period of absence.
Notification of Employer. 1. An employee who receives a jury summons shall notify his or her supervisor not less than ten (10) working days prior to the first day of actual service. Failure to do so shall disqualify the employee for benefits under this Article.
2. With respect to any jury summons received by an employee the Company reserves the right to request exemption from jury service for such employee.
Notification of Employer. Respondent shall provide his present and any future employer a copy of the Notice of Hearing and Statement of Charges, and this Settlement Agreement and Final Order. Respondent shall within ten days of the date of this agreement provide the Board with written verification from his current employer this notification has occurred. This written verification shall be provided to the Board within ten days of employment with any new employer.
Notification of Employer. Employees shall provide the Employer with two (2) hours notice prior to using PTO.
Notification of Employer. An employee on banked sick leave shall inform his/her supervisor of the need to use banked sick as soon as possible. When applicable, the employee will be required to obtain, complete and return required documentation to the Human Resources Department for approval of the leave. Failure to do so within a reasonable time may be cause for denial of sick leave with pay for the period of absence. Youth Center employees are required to provide four (4) hour notice prior to the commencement of their shift.
Notification of Employer. An employee requesting bereavement leave shall provide written notification to their supervisor within 24 hours of requested leave. Failure to do so may be cause for denial of bereavement leave with pay for the period of absence.
