Notification of Unscheduled Absence. Employees who are unable to report for duty because of illness or any other reason shall call the on duty Battalion Chief or their immediate supervisor one hour prior to start of duty or notify Telestaff in accordance with department policy. Failure to do so shall be basis for discipline.

  • Notification of Absence To be entitled to payment in accordance with this clause the employee shall meet the following criteria;

  • Application for Leave of Absence 25.01 All leaves of absence without pay and any extension thereof must be applied for in writing to the Mayor or his designee, on forms supplied by the Employer, at least ten (10) working days prior to the proposed commencement of the leave except in serious or unusual circumstances, as determined by the Employer.   • Medicaid Notification of Termination Requirements Party shall follow the Department of Vermont Health Access Managed-Care-Organization enrollee-notification requirements, to include the requirement that Party provide timely notice of any termination of its practice.

  • Leave of Absence for College Committees An employee whose assigned work schedule would prevent her/him from attending meetings of a college committee to which s/he has been elected or appointed, will be granted a leave of absence from her/his regular duties without loss of pay or other entitlements to attend such meeting(s). Where such leave is granted, the employer will replace the employee as necessary. Costs arising from this provision will not be charged against the program area of the participating employee.

  • Extended Absences In the event Tenant will be away from the Premises for more than 7 consecutive days, Tenant agrees to notify Landlord in writing of such absence.
