Orthodontic. Default level, if there are dependent children, covers dependent children only to a lifetime limit of $2,000 per child.
Orthodontic. The Board agrees to contribute ninety percent (90%) towards the premium of 50% co- insurance for orthodontics to a lifetime maximum of $500.00 for active employees, adults and children.

The orthodontic plan covers all family members, including employee and spouse with a $4,000 orthodontic lifetime benefit on an 80%/20% co-payment program. The City reserves the right to change or add orthodontic plan providers during the term of the MOU. The City will notify the Association regarding any change in carriers. The City reserves the right to propose plan modifications during the term of this MOU. Any Employer proposed changes in the plan coverage or co-pays should be implemented by mutual agreement.
Orthodontic. The company will pay fifty (50%) of the premium cost for Orthodontic coverage for all employees and their dependents commencing on the first day after completion of six months of employment. Reimbursement under Orthodontic coverage is fifty (50%) of insured charges with a total maximum payout for any eligible member of one thousand five hundred dollars ($1,500.00) per life time.
