Placement of Materials in File. 1. Routine information concerning status, pay, evaluations and the like are placed in the file by the District Office staff. 2. Other information, including letters of commendation, may be placed in the file by the Supervisor or the Superintendent or designee. Disciplinary materials will be placed in the file in accordance with the procedures set forth in the discipline policies. 3. The employee is responsible for placing copies of job related licenses and certifications. 4. Material placed in file shall be dated.

Placement of Materials in File. Any person who places written material or drafts written 2 material for placement in any employee’s file shall sign the material and signify the date 3 on which such material was drafted. Placement of Materials in File. Only one official file shall be maintained. No evaluative materials shall be placed in the file unless the employee has had an opportunity to read such material. The employee shall acknowledge that he/she has read any evaluative materials by signing the copy to be filed. The employee's signature shall mean only that he/she has received and read the contents therein. Any evaluative material not reduced to writing within thirty (30) calendar days following the event or 30 calendar days after the employer’s knowledge of the event may not be added to the file. Any materials not contained in the employee's personnel file may not be used to evaluate or discipline the employee in any manner. Upon request, the employer will reproduce any materials in his/her personnel file.
Placement of Materials in File. The District shall maintain one (1) official personnel file for each employee. The District may maintain separate files containing medical records and other personnel documents as authorized by law. Employees shall be provided with a copy of any document regarding an employee’s performance upon its placement in their personnel file. The employee shall acknowledge receipt of any such material by affixing his signature and date on a copy which the employee shall return to the supervisor. The employee’s failure to acknowledge receipt of any such document shall not preclude its placement in the file.
Placement of Materials in File. The person who places non-routine material in a unit member's personnel file shall sign the material and signify the date on which such material was placed in the file. a. A unit member shall be provided a copy of any negative or derogatory material when it is placed in his/her personnel file and be given an opportunity to initial and date the material and to prepare a written response to such material. The written response shall be attached to the material. b. The District shall not suspend or terminate a unit member based upon materials which are not contained in such unit member's personnel file unless mandated by law. c. Documents in personnel files shall be removed if determined to be inaccurate. Such determination shall be sustained through the Grievance Procedure. Specifically exempt from such procedures is Section 10.1 and all other documents which have legal or administrative remedies.
